Who is a Nursing Capacity Planner?
The Clinical Planner is accountable for assessing current patients requiring nursing services within an assigned district, through the assessment, implementation, and evaluation of a wide range of clinical nursing interventions.
What you will be doing:
- Perform mobility assessments to determine the most appropriate setting for ongoing care services (Clinic, Community)
- Complete episode of care and assessment of care plan, and frequency of care provided
- Complete the safety assessment of a patients’ living environment to ensure employees will be working in safe and healthy work conditions.
- Advise Operations of the appropriate parameters for scheduling the patient’s ongoing visits, based on clinically indicated service requirements.
- Inform patients and families of ParaMed policies and scheduling practices, as well as the expected on-going care setting and/or nursing visits.
- Maintain current and accurate patient record, and audit of clients on-going charts
- Show respect for cultural needs, dignity, privacy, and confidentiality of patients and staff.
- Stay abreast of changes in practice setting that affect scope of practice.
Experience you bring:
- Registration in good standing as a RPN/LPN with provincial governing bodies and minimum two years’ experience as a practicing nurse.
- Working knowledge of applicable legislation including but not limited to Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / Personal Health Information Act, Occupational Health and Safety legislation and practices.
- Knowledgeable in best practice standards for community health care and services.
- Strong oral and written communication skills
- Excellent organizational skills
- Demonstrated competency in patient centered care principles and practices.
- Knowledgeable of community resources available for patients and families.
- Strong independent critical thinking and problem-solving skills.
- Ability to provide a current, clear criminal back check with Vulnerable Position Screening (VPS)
- Access to a personal vehicle required
