The Office of Population Health Management and Health System Partnerships strengthens LHSC’s commitment to work with patients, families, caregivers, and partners from across the system to support the health of all members of the populations we serve. Following the lead of health systems around the world, population health management (PHM) has been identified as a key component of Ontario’s health system transformation. PHM is grounded in the idea that, to develop an effective healthcare system, we must start by accepting collective accountability for the health and wellbeing of everyone in our population. Together with system partners, we must then strive to address their health needs through participation of, engagement with, and targeted interventions for individuals within those populations. The Office has established five areas of focus including health system coordination and governance, care design, change management, population level data, analytics and technology, and research, innovation and education.

Job Summary

What the Role is

The Director is accountable to provide oversight and leadership for the development of innovative care delivery models (at the system level) using a partnership approach to support improved population health management and promotion of the health equity driven quadruple aim related to: care pathway development with regional partners; oversight of the converting the output into Business Process Modelling and Notation (BPMN) diagrams and simulation models; leading change management strategies, ensuring strategic alignment of change initiatives at organizational, local, regional, provincial, and international levels; applying project portfolio management; supporting health system transformation through Ontario Health Teams and changes in government policy; leading co-design and managing regional stakeholders towards integrated care; process improvement skills and business acumen.


Reporting to the Senior Director, Health System Partnerships, the Director, Care Design & Change Management will translate decisions of our senior level into execution by utilizing systems thinking strategic planning and relationship management by developing purposeful coalitions with internal and external stakeholders. This position requires someone with a proven track record for leading and driving systems transformation through encouraging and supporting innovation and championing and orchestrating change, developing and sustaining healthy work environments, and effective teams while leading in a performance-driven culture. You will organize program activities, direct resources, execute operational initiatives, formulate policies & procedures, and strategically plan for the future.


You will demonstrate strong business acumen including the ability to manage within multi-million dollar operating and capital budgets. This leader will have exceptional analytical and critical thinking skills that contribute to identifying problems and issues, evaluating the interdependence of decisions, questioning and challenging the status quo, and designing and implementing processes for improvement.

What You Will Bring with You

  • Successful completion of a recognized Master’s Degree in business or health profession required
  • Trained in and experience with Business Process Modelling and Notation (BPMN) diagrams and simulation models
  • Working knowledge of multiple quality improvement and performance measurement methodologies including LEAN, Six Sigma
  • Working knowledge of and experience with change management and co-design methodologies including patients, care partners, and broader system of partners
  • Thorough understanding of how to enable the culture shift that will be required to enhance the patient experience and engage in quality improvement initiatives
  • Minimum of 8 to 10 years’ leadership experience, preferably in progressively more responsible leadership position(s)
  • Minimum five (5) years' previously related demonstrated experience and understanding in a healthcare-related discipline, preferably in areas system change, patient experience, quality improvement, and performance management
  • Previous leadership in Population Health Management & Health System Partnerships is preferred
  • Member of the Canadian College of Health Leaders (CCHL) who have, or are in process of attaining the Canadian Health Executive (CHE) Select certification program preferred
  • Fluent and computer literate with computer systems such as email and Microsoft Office applications (MS Word, Excel & PowerPoint)
  • Demonstrated ability to attend work on a regular basis

In addition, candidates will need to provide documentation of Tuberculosis Skin testing and a completed COVID vaccine series (two vaccines).
