The Public Health Nurse (PHN) is an integral member of a multi-disciplinary team, who will promote health within the community.
The PHN is responsible for the overall leadership of initiatives and activities through expertise in the six critical program areas of
prevention, education, case management, support, and research. The PHN will create interdisciplinary collaborative relationships
with fellow health care team members and other community agencies to ensure a high standard of practice and delivery of quality
client care.
Guided by the Northern Health Region (NHR) Primary Health Care principles, this position will strive to improve the health status of
the residents of the NHR working from an advanced understanding and knowledge of disease management, harm reduction,
population health, determinants of health, and case management delivered with a primary health care framework. The PHN will
develop and implement health plans from a client centered strength/holistic approach.
Specifically, the Families First Coordinator shall facilitate the coordination, planning, implementation, and evaluation of the Families
First Program in accordance with the guidelines set forth by Healthy Child Manitoba and in keeping within the provisions of the
Vision, Mission, Values, and Policies and Procedures of the NHR.
QUALIFICATIONS
• Baccalaureate Degree in Nursing required
- Current active practicing registration with the College of Registered Nurses of Manitoba (CRNM)
- Current certification from a recognized Healthcare Provider Basic Life Support (BLS), or obtain within three (3) months of
- Must have a valid Class V driver’s license, access to a vehicle, and be willing to use it to travel in region throughout the year
- Certification in Community Health Nursing through the Canadian Nurses Association is preferred
- Expectation to complete the skill enhancement modules through the Public Health Agency of Canada
- Familiar with Windows based programs (Microsoft Word, Excel, Outlook) and Internet
- Satisfactory employment record required
