Description
DUTIES AND RESPONSIBILITIES:
Provide case management services for all assigned children, working with other family support providers for guidance and assistance.
Coordinate the provision of services to clients including consultation, assessment, therapy, group composition/scheduling and follow-up for all clients relevant to the service in accordance with the programs mandate, policies, and procedures.
Assess child and family strengths and needs, including risk identification, according to program standards.
Ensure the continuous implementation of the case management system and implement regular, appropriate documentation and written reports, including data collection and data reporting, for all clinical files in keeping with the standards of CFW.
Develop treatment plans and goals in conjunction with the family, addressing risk management.
Carry out various treatment components outlined in case management and treatment plans, including but not limited to: individual, family, and group counselling with parents and child, effectively liaise, consult, and collaborate with other service providers (e.g., police, schools) and make appropriate use of community resources, act as an advocate when needed (within the school and child welfare systems);
Conduct face to face interviews with each family and child (individually) at least one time per month or more often if needed.
Maintain familys confidentiality per Zagim Anishinabk standards and all applicable codes of ethics.
Work within the following environments: home, community, school, on-reserve school.
