Casual Continuing Care Assistant- All Shifts

Main Activities

There are two homes- one which provides transition, respite and palliative care. The other which provides a program to at risk moms who are prenatal and postnatal preparing them for transitioning to a new home with their infants.

· Provide supportive care to people living with or at high risk for HIV/AIDS who may also be affected by mental illness, addictions, poverty, homeless and other psycho social factors

· Provide supportive care through a patient centered model aimed at creating independence and autonomy while fostering an environment of acceptance and understanding

· Assist with and support clients with medication adherence

· Work with patients on individual goals and care plans aimed at recovery and management of their illness(s)

· Provide therapeutic counselling/assistance to clients who may present with multiple issues affecting their ability to cope and manage in the community

· Advocate on behalf of clients for additional service and resources, as required

· Personal care to patients including, assisting with grooming, bathing, personal hygiene, toileting, wound care, nutrition and all other activities associated with daily living

· Establish and maintain current, accurate, confidential files for each client

· Educate clients, families and support systems about Sanctum and what can be provided and when to access other resources

· Assist with the transition plans from Sanctum into the community set by the client and care team

· Follow medical instructions carefully and consistently

· Assist with meal preparation and planning, as well as other house hold duties such as cleaning and maintenance

· Organize time and resources based on the individual needs of clients

· Manage any unanticipated events or unstable situations

Job Type: Casual

Salary: $18.00-$23.00 per hour
