The Community Outreach (On-Reserve) worker is responsible for providing a broad range of support, education, and crisis intervention services to individuals affected by (or likely to be affected by) interpersonal violence with the goal of guiding clients to a path of self-sufficiency that includes transfer of knowledge, strength, healthy habits, tools for change, and the confidence to make informed choices.
JOB DUTIES AND RESPONSIBILITIES:
Utilizes identified assessment and case planning tools to assess clients support needs and determine case plan;
Establishes a clear case plan, and works with clients to set goals and monitor the progress towards those goals;
Assists clients in setting up appointments and filling out forms in relation to their needs;
Identifies required supports and other critical client needs (i.e., Childcare) for clients to meet the goals of their case plan;
Offers transportation to necessary appointments;
With the clients permission, provides referral services to supports that cannot be offered internally;
Builds and maintains a network of positive working relationships with other community service providers, community groups, etc. to develop programming, coordinate program delivery, ensure effective referrals and a good continuum of care;
Other work-related duties as assigned by Immediate Supervisor.
JOB REQUIREMENTS:
Completed post-secondary education, preferably in Social Work, Human Justice, or another relevant human services field;
At least 2 years' experience in a human services field;
A combination of education and experience may be considered;
MUST possess a Valid Saskatchewan Driver's License;
MUST be able to provide a clean Vulnerable Sector Check;
Ability to speak Cree and/or Dene is considered an asset;
Completed relevant training such as Safe Talk, ASIST, Mental Health First Aid, Sexual Assault First Responder, Motivational Interviewing, etc.
