Position Description

Reporting to the Justice Programs Manager, the Community Reintegration Pathways Specialist will support Indigenous adults as they reintegrate into the community after release from custody. This position will support area First Nations members through psychosocial rehabilitation with a restorative and recovery focus, by providing reintegration and recovery supports.

Department: Development Services for Child and Youth

Term: Full-time

Location: 240, Veteran’s Drive, Kenora, Ontario

Reports to: Justice Programs Manager

Responsibilities

Ø Provide Indigenous, client-centered case management services to those coming out of the correctional institution

Ø Develop relationships with First Nations communities, Ministry staff, community justice staff, KCA staff and community partners

Ø Participate in Community Reintegration tables and encourage client participation in discharge planning

Ø Develop a reintegration plan prior to discharge

Ø Create and maintain client file, document services, supports and referrals, including KCA services.

Ø Meet with clients regularly (in custody and post-release) to assess, refer, connect and support them, and to monitor their progress

Ø Assist in connecting clients with their communities and community support services pre-discharge and support clients in community post-release

Ø Facilitate connection to the land through land-based healing and provide culturally relevant supports

Qualifications

Ø Post-secondary education in the mental health, social services and/or justice fields or equivalent combination of training and experience

Ø Minimum 3-5 years of related experience

Ø Strong time management skills

Ø Ability to work independently and in a team setting

Ø Skillful in the following areas: computer, EMR, organization, interpersonal, presentation and communication (written and oral)

Ø Experience working with and knowledge of Indian Residential School survivors

Ø Knowledge of the Anishinaabe culture and language and experience in working in an Indigenous community or organization considered an asset

Additional Requirements

Ø Valid driver’s license and vehicle

Ø Willing to travel according to program needs

Ø Current satisfactory Vulnerable Sector Check

Ø Obtain and maintain security clearance

Ø Understanding of the Ojibway language is an asset
