MLTC Health & Social Department is seeking to fill a part time, permanent position of Diabetes Nurse Educator. The Diabetes Nurse Educator reports to the MLTC Nursing Program Manager and plays an essential role in two components of the MLTC Health Service Delivery Plan: The MLTC Home Care & Chromic Disease Management. The Diabetic Nurse Educator work with each of the eight (8) Meadow Lake First Nations to continue to achieve optimum care for the MLFN members who are admitted to our Home Care program and Chronic Disease Management programs.
