SalaryThe North Lambton Community Health Centre is a community governed health centre serving identified priority populations in a defined catchment area. The Centre operates multiple sites in Ontario: Forest, Kettle & StonyPoint, Watford, and Sarnia. Our interdisciplinary team provides primary health care, health promotion and community development services.
The North Lambton Community Health Centre is currently seeking applicants for the following position:
HEALTH COACH
18 Month Contract
30 Hours/Week
The Health Coach will work mainly with the Cardio-Pulmonary Rehabilitation Team to assist individuals and groups with chronic disease self-management though the delivery of behaviour modification, education and exercise programs.
Qualifications:
- High school diploma or greater.
- Post-secondary degree/diploma in a fitness or health-related field.
- Senior Fitness Training, or willingness to obtain.
- Previous experience working in Health Promotion and Fitness settings.
- Experience educating adults and instruction of behaviour modification programs.
- Demonstrated ability to work in an interdisciplinary collaborative practice team environment.
- Proficiency in the use of computers and various software applications.
This is a part-time contract position, working 30 hours per week. Monday to Thursday at the Cardio-Pulmonary Rehabilitation Site in Sarnia. The rate of pay for this position is $22 per hour plus 10% in lieu of benefits. The Healthcare of Ontario Pension Plan is also available.
