Job description

The Position

Reporting to the Health Manager, the Home Health Liaison will provide comprehensive ongoing case management services to complex geriatric clients and adults with chronic and acute illness. The successful applicant will work with clients and families to address health, psychosocial, environmental and functional needs to maximize recovery from or adaptation to illness or disability. Also, you will facilitate access to community services and support in the community. This is a one-year full-time and termed position.


The Candidate:

  • Oversee the coordination of home support services and home cleaning services for clients, supporting the planning and delivery of home support services.
  • Support the coordination of home care on a daily, weekly and monthly basis to create the most time-efficient and cost-effective schedule to meet the needs of the clients.
  • Support home support workers and homemakers to address issues and concerns to create safety and accountability.
  • Work with external care providers to ensure that Nation members have access to appropriate supports and resources such as VCH Homecare.
  • Work with the Health Manager to control and prioritize utilization of home care staffing resources to reflect actual client home care needs.
  • Effectively communicate and coordinate with internal staff and clients and their families regarding home care needs, concerns and/or problems with coverage.
  • Record, maintain, monitor and verify accurate home care records, including service documentation including follow-up reports with the client and health team.
  • Assist with obtaining and tracking equipment, supplies and services such as durable medical equipment, home-delivered meals, medical response systems and incontinence supplies as reflected in the plan.
  • Supporting nursing with client assessments and liaising between care facilities and community services to facilitate discharge planning from acute care to community.
  • Perform other duties within the scope of the position, as assigned.


Qualifications, Skills and Abilities:

  • Certified as a Home Care Aide.
  • An undergraduate degree or college diploma in a social science or related discipline is desirable.
  • Knowledge of shíshálh Nation programs and services or willingness to learn.
  • Respect for, sensitivity towards, knowledge, and understanding of shíshálh culture and traditions.
  • Knowledge of relevant legislative frameworks and policies reflecting current youth welfare practice.
  • The ability to translate relevant legislation into the Nation language, policies and procedures.
  • Knowledge of First Nation service delivery, customs and traditions.
  • Knowledge of external service and service agencies.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to develop and implement a client care plan, including follow-up.
  • Current BC Driver’s License and access to a personal vehicle.
  • Acceptable Criminal Record Check with Vulnerable Sector Search.
  • Physical ability to carry out the duties of the position.
  • Proficient in the use of MS Office programs with advanced knowledge of Excel and strong database skills.
