Job description

This is a contract position for up to 2 years. $23.00 hourly for 37.5 hours per week with benefits option after 3 months. Additional work benefits and responsibilities may include local travel allowance to support clients, work cell phone, and access to funds to directly support housed clients. Staff are required to work on-call shifts throughout the month and will be compensated accordingly.

1. Supervision: Employee performs duties within established policy guidelines goals and objectives, and has primary responsibility for the delivery of an area of work. The employee works under the direction of the HOPE Team Coordinator and supervision of the Executive Director.

2. General Summary & Competencies: The HOPE Housing Support Specialist contributes to the overall success of the community goal of ending chronic homelessness and ensures a successful Housing Support program for the agency. This is accomplished by effectively completing various tasks, relating to ensuring sustained tenancy, and client-led intensive case management. Self-directed and enthusiastic, the HOPE Housing Support Specialist is responsible for meeting outcomes as identified and directed by the agency. This is a direct service position providing ongoing support as part of a collaborative team for individuals housed off the By Names List who have a history of homelessness and present with multiple barriers to housing resulting in the requirement for supported housing.

3. Essential Duties of Position: Employee advances the vision and mission of Fresh Start Services.

Program Delivery:

  • develop individualized case management plans that build participants ability to maintain housing.
  • Assist individuals with identifying barriers to housing stability.
  • Assessing readiness for change and client-led strength-based approach to identifying areas in need of development in the living, learning, working and social environments as appropriate and support clients in their personal goals.
  • Assisting clients in obtaining income, health services and other benefits, ID etc. as needed
  • Position may require transportation of client in personal vehicle.
  • Maintain communication with HOPE Team and other community agencies involved.
  • Ensure all program forms and documents are completed, filed, and kept up to date.
  • Address any challenges/issues that may present themselves
  • Relay of appropriate information to staff.
  • Home visits, apartment checks
  • Client referral to various services
  • Develop client-led strategies for sustaining and growing housing stability
  • Problem-solving and critical thinking as it relates to challenges that present for the client.

Data Collection and Reporting

Manage and track daily/weekly (as needed) scheduled visits and ensure case notes are documented properly.

  • Keep and compile statistical data for Fresh Start and the CAS as required.
  • Enter data into HIFIS.
  • Ensure that short-term and long-term project objectives and outcomes are met, as delineated in the funder’s agreement.

Additional Duties

  • Effectively and appropriately use technology and social media to meet project outcomes.
  • Participate in agency-related training, outreach sessions, initiatives, or events.
  • Other duties may be requested/required as part of CAS.

4. Special Conditions of Employment:

  • Must have successfully completed vulnerable sector/child abuse registry checks; must be willing to do outreach within Saint John to
  • Must be open and non-judgmental in working with vulnerable clients living with homelessness, mental illness, poor mental health, substance use disorder and living with generational poverty and the effects of trauma.
  • Must have an understanding and acceptance of harm reduction and trauma informed practice.
  • Post Secondary Education in combination with professional experience; lived experience is an asset.
