Job description

Job Summary: Provide emergency shelter case management, community meal program assistance, data program maintenance, Participant engagement and safety, reporting, and fostering community partner relations, under the direction of the Day Program Supervisor.

Responsibilities:

Emergency Shelter Case Management

  • Practice strong cohesive diversion techniques on an ongoing basis.
  • Assist with the facilitation of the motel voucher program.
  • Coordinate access to emergency shelter as required.
  • Use Case Management as a tool to support Participants in identifying their unique housing barriers, and set goals to achieve, change, and secure housing in conjunction with Emergency Shelter and Community Partner teams.
  • Support Shelter Staff to encourage Participants in reaching housing and related goals and ensure accountability to housing expectations to maintain emergency shelter use.

Community Meal Program Assistance

  • Identify and support those who are experiencing homelessness or at risk of homelessness using the community meal program or other Lighthouse services.

Data Requirements

  • Participate in Built for Zero and Coordinated Access related meetings.
  • Compile service delivery statistics, input required data, and run statistical reports as needed.
  • Assist with the development and implementation of new data collection systems.

Participant Engagement and Safety

  • Maintain an emotionally, mentally, and physically safe environment for the Participants.
  • Assist Participants in building positive relationships that are healthy and supportive.
  • Establish and maintain professional boundaries with Participants.
  • Safely resolve crises and respond nonjudgmentally to problematic behaviour.

Reporting

  • Complete accurate, professional, factual, and non-judgmental incident reports, client records, and case management notes.
  • Perform assessments using SPDATs to assess needs and appropriate program advocacy further.

Community Partners Relations

  • Initiate new and maintain existing community partnerships that support housing and other areas to meet the needs of the Participants.
  • Assist Participants in connecting to appropriate community resources for aftercare to maintain housing and act as an entry point for re-engaging aftercare supports.
  • Help make appointments with community partners, such as the Clinic.

Administrative Duties

  • Provide administrative support by phone, fax, and email as needed
  • Take part in internal department collaboration and help to develop policy and procedures.

Perform other duties as assigned.

Qualifications:

  • College diploma or University degree in business, human services, or equivalent
  • 2 years experience in related field, including any post-secondary co-op placements or internships.
  • High standard of integrity, judgement, and ability to maintain confidentiality.
  • Highly organized, analytical, thorough and detail-oriented.
  • Experience in database management.
  • Ability to multi-task and manage several projects at once.
  • Effective communication skills, both written and verbal.
  • Solid computer and internet skills; Microsoft Office.
  • Professional or personal experience with those experiencing homelessness.
  • Excellent customer service, problem-solving, and decision-making skills.
  • Flexible and adaptable, with compassion and sensitivity to the issues related to poverty, homelessness, mental illness, addictions, and developmental disabilities, with a commitment to a discrimination-free environment.
  • Ability to work independently and as part of a team.
  • Proven experience to work independently and as part of a team.
  • Willing to adhere to the vision and mission of The Lighthouse.
  • Clean criminal record check, including vulnerable sector screening.
  • Valid Ontario driver’s license, liability insurance, and access to a reliable vehicle.
