POSITION PURPOSE

Reporting to the Supervisor, Community Stabilization Programs, is responsible for assessing and verifying eligibility, delivering financial assistance and providing follow-up supports through a wraparound service delivery model to Regional clients on Ontario Works and the Ontario Disability Support Program (ODSP) as part of the Housing Stability Program (HSP), including referring residents to appropriate training, employment, health, financial literacy, housing, social services and community programs.


MAJOR RESPONSIBILITIES
  • Interviews applicants to assess and determine eligibility for assistance under the York Region’s Housing Stability Program Guidelines.
  • Verifies information provided by the applicant to confirm eligibility.
  • Determines available financial assistance and other potential income sources that may be available to applicants.
  • Provides case management through an initial assessment and through follow-up contacts with clients.
  • Provides a wraparound service to support clients to promote Housing Stability.
  • Develops a case plan and sets goals with clients issued HSP funds.
  • Maintains files and provides case administration in compliance with the appropriate Acts and Regulations.
  • Complies with York Region Housing Stability Program Guidelines and documents recommendations for exceptions for Supervisor approval, as required.
  • Seeks out Regional and external client supports.
  • Responds to inquiries from internal and external customers regarding services and programs.
  • Performs other duties as assigned, in accordance with Branch and Department objectives.
  • May occasionally be required to respond to emergency situations outside of normal business hours, as related to the Departmental Emergency Plan.

QUALIFICATIONS
  • Successful completion of a Community College Diploma Program in Social Work, Social Science, or a related program.
  • Minimum one to three (1-3) years experience in a social service environment, including experience with disadvantaged individuals, homelessness, mental health and senior population, employment counselling and group/workshop delivery and demonstrated skills to work in a demanding environment.
  • Valid Ontario Class “G” Driver’s License and reliable vehicle for use on corporate business.
  • Satisfactory Criminal Records Check.
  • Knowledge of industry-related legislation and regulations applicable to the program delivered (e.g. Ontario Works Act or Ontario Disability Support Program Act).
  • Knowledge of mental health, addictions, counselling, financial literacy, employment and housing programs and services.
  • Knowledge of issues and barriers related to the challenge of maintaining Housing Stability for families and individuals.
  • Knowledge and demonstrated ability in corporate core competencies including customer focus, communication, collaboration and personal ownership.
  • Ability to compile statistical data and reports for the purposes of review and evaluation.
  • Ability to travel to off site locations in a timely and efficient manner, as required.
  • Ability to work outside regular business hours, as required.
