Job description

HOURS: DAYS & WEEKENDS

SALARY: $37.93 (MIN) – 54.37 (MAX)

The Patient Flow Navigator will support the philosophy that each patient receives the right level of care in the right place at the
right time by the right provider. Responsibilities of this position include but are not limited to:

Promoting optimal patient and clinical outcomes by assessing and facilitating the effective management of care,
utilization of resources and length of stay

Addressing identified processes and/or other issues impeding the patient’s flow from admission to discharge.

Coordinating the multidisciplinary team‘s plan of care to make sure needs are met in a timely manner and consistent
messaging to the patient and family regarding discharge plans is ensured

Building awareness of patient navigator role/services among the health care team is important to assist in coordinating
patient care and identifying “at-risk” patients that need navigation services.

Coaching patients to become advocates for their own care

Empowering patients to self-navigate the healthcare system internally and externally

QUALIFICATIONS:
Current Certificate of Registration ~ College of Nurses of Ontario required

Minimum of five (5) years of recent acute care experience required

Current Non-Violent Crisis Intervention (NVCI) Certificate required (training delivered by Health Centre upon hire)

Current Basic Life Support (BLS) for Health Care Providers (HCP) Certificate required (recent within the last twelve
months)

Extensive knowledge gained through in-depth involvement in a complex technical/professional field and/or broad
experience in several fields required

Demonstrated ability to work collaboratively in a multidisciplinary team environment including internal and external
partners required

Demonstrated strong leadership ability required

Demonstrated facilitation, critical thinking and problem solving skills required

Demonstrated exceptional organizational skills required

Data collection and analysis/accuracy experience required
Demonstrated leadership skills and ability to apply conflict resolution skills required

Ability to rapidly shift workload priorities

Ability to provide leadership in emergency situations

Demonstrated ability to be self-directed and thrive in a dynamic environment

Ability to support continuous quality improvement initiatives

Demonstrated knowledge of various bed types within the NBRHC and ability to recommend bed allocation based on the
individual admission criteria unique to each unit (i.e. Acute Surgery/Medicine, Complex Continuing Care, Rehabilitation,
Assess-Restore Beds, Mental Health, etc.)

Good attendance and work record required

Must demonstrate commitment to ensure your safety, the safety of fellow workers, patients, volunteers and visitors

Must support and contribute to a culture of safety and prevention of adverse health events in this organization

Bilingualism (English/French) is an asset

Criminal Reference Check including the Vulnerable Sector Check recent within six (6) months is a requirement
