About our Hospice:
Lisaard/Innisfree Hospice brings together an interdisciplinary team of trained professionals to deliver services that can make the dream of a peaceful, comfortable death surrounded by loved ones a reality and to fulfill the mission of hospice care by supporting the residents and their family through the end of life journey.
Accepting direction from the Registered Nurse (RN), and under the supervision of PSW Lead and the Director of Clinical Services, the Personal Support Worker is an integral member of that interdisciplinary health care team providing quality and meaningful palliative care.
Hours of Work:
Flexible schedule within a 24 hour/7 organization, working at both hospice sites. 12 hour shifts
Casual employees are required to pick up 1 shift in a 4 week schedule.
Responsibilities include, but are not limited to:
· Acts within his/her scope of practice by following established care plans and the policies and procedures of Lisaard/Innisfree Hospice.
· Performs both personal care tasks and incidental activities of daily living such as housekeeping, meal preparation, socialization and companionship.
· Understands the philosophy of hospice palliative care and has specialized education and/or experience in providing care to palliative clients.
· Participates in interdisciplinary committee work and projects as required.
· Accountable for maintaining a high level of confidentiality and privacy standards
· Uses therapeutic communication by establishing a working relationship with the residents, families and coworkers for the purpose of meeting the resident’s needs ensuring respect, dignity, and individuality of all.
· Participates in continuing education relevant to palliative care to ensure skills and abilities are maintained and enhanced.
· Maintains a safe work environment by adhering to all health and safety policies and procedures and ensures co-worker practices are also in compliance with the Ontario Health and Safety Act (OHSA).
Requirements:
· Graduate of recognized Personal Support Worker program
· Maintains CPR/AED certification annually
· Minimum of 1 year palliative care experience; new graduates with student experience in a palliative care setting will be considered
· Some community experience is an asset
· Completed the Fundamentals of Palliative Care Course and subsequent Advanced Practice Skills course or completion within 1 year from their date of hire
· Completed the Food Safety Training Certification offered at Conestoga College or equivalent or completion during probationary period
· Must possess basic computer literacy and willingness to learn the computer documentation used by the organization
· A Police Vulnerable Sector Check (PVSC) is required prior to the start date
· Must be fully vaccinated against COVID-19 (minimum of 2 doses).
