JOB SUMMARY:
The Psychosocial Worker collaborates as a member of the interdisciplinary team and delivers comprehensive psychosocial services to clients/residents and their families. The psychosocial worker uses a variety of interventions including psycho-social assessments, resource linking, and clinical counseling to address social-emotional issues, bereavement, transition and adjustments due to chronic illness and disability resulting in residential placement. Provides ongoing education and consultation to clients/residents, families and healthcare professionals for care planning. Maintains community resource links.
SPECIFIC DUTIES AND RESPONSIBILITIES
- As a multidisciplinary team member, participating in ongoing care planning, involving the resident and family
- Leads the resident move in, transfer, and move out process in accordance with the organization’s move in experience standards to include: facilitation of the application process including assistance to access financial supports as required, tours, introducing the resident/family/substitute decision maker (SDM) to the community and its operations, assists and supports resident/family/SDM with adjusting to living in residential care
- Enhance residents’ care, including promoting holistic and cultural approaches, maintaining therapeutic relationships with residents and families and providing specialized individual and family counseling, engaging in resident advocacy, family support groups, family councils, and resident councils, and coordinating with other community services;
- Assist in the development of communication strategies for residents and their families
- Conduct psychosocial, cognitive, depression, autonomy and observation assessments
- Provide mediation between ALTCC and resident/family
- Ongoing education for residents, family and staff about the importance of person centered/social and cultural model of care
- Active engagement with outside agencies to offer additional support or augment what already exists
- Working the clients and the team proactively to solve interpersonal or behavioral issues a resident may be experiencing
- Provide psychosocial expertise in aspects of care
- Provide supervision and guidance to the social department as well as directly oversee and support the activities coordinator
JOB QUALIFICATIONS
Education & Experience Requirements
· A post-secondary degree in social work, psychoeducation, psychology and/or other related field
· One year's recent, related experience or an equivalent combination of education, training and experience
Knowledge
· Knowledge of regulatory standards of care or best practices in long-term care.
· Knowledge of issues related to powers of attorney, substitute decision makers, protection mandates and wills
· Knowledge of health and social services in our region
· Knowledge of proper record keeping and resolution of ethical and/or confidentiality issues
· Understanding the need for cultural safety
· Satisfactory criminal reference check including Vulnerable Sector search
· An appreciation of indigenous culture and traditions
Job Type: Casual
Salary: $25.63 per hour
Expected hours: 1 – 40 per week
