Position Title:Program Activationist

Immediate Supervisor: Manager of Community Supports and Program Coordinator Day Services

Hours of Work: Part Time (20 Hours per Week)

Summary

Reporting to the Program Coordinator Day Services, the Program Activationist will implement, monitor, and evaluate social, recreational, restorative and activation programs for seniors and adults with disabilities. The role delivers flexible support activities that meet the needs of our clients through a variety of planned activities. Clients may have a range of support needs associated with learning disabilities and physical disabilities.

RESPONSIBILITIES

Delivering the Service

  • Assist with planning a range of recreational, learning and life skills activities, and facilitating the delivery of planned activities.
  • Obtain client input in service design in order to develop options that meets their needs, aspirations and interests.
  • Supporting clients to identify personal goals, outcomes and support needs, and reviewing these on a regular basis.
  • Work in collaboration with the Manager of Community Supports and Program Coordinator Day Services to share information and promote continuous improvement of client centered activities.
  • To assist clients with their mobility including the use of aids such as wheelchairs.
  • Represent the Community of Support Centre of Essex Count (CSC) externally in a professional manner.

Meeting Client Needs

  • Seek opportunities to involve clients in changes to service design.
  • Ensure all interaction and communication is appropriate to the individual’s needs.
  • Champion and support clients to enable them to integrate within and be part of their local communities.
  • Facilitate and have professional working relationships with families and support networks, health and social care professionals and agencies, communicating effectively

Policy and Procedures

  • After induction and training, ensure a working knowledge of all policies and procedures that are relevant to the role, e.g. equality and diversity, health and safety, data protection, safeguarding. Completing ongoing training and refreshers as required.
  • Complete necessary administrative tasks including ensuring that all relevant documentation is completed correctly and maintain all records and systems as appropriate.
  • Report all complaints from clients and/or their representatives.

Safeguarding

  • To safeguard the health, well-being, and safety of the clients we work with, some of whom may be classed as vulnerable people or adults at risk. In the event of a risk to a client becoming apparent or if concerns arise about a vulnerable person’s welfare, to immediately report these concerns in line with the appropriate policy and procedure.

Health and safety

  • To lead health and safety within client activities by setting clear examples of good practice, and ensuring they report accidents.
  • Work within the organizations health and safety policies and procedures
  • In addition to these responsibilities, employees are required to carry out such other duties as may reasonably be required.

QUALIFICATIONS & REQUIREMENTS

  • Practical experience delivering learning and physical activities to seniors and adults with a disability
  • Experience in the Not-For-Profit Community Support industry preferred.

Skills/Abilities

  • Able to work with people with mild to moderate learning and physical disabilities.
  • Able to take a positive and active support approach to working with seniors and disabled people, enabling them to achieve personal goals.
  • Able to communicate clearly and adapt approach to ensure customer participation
  • Able to monitor, review and evaluate client progression
  • Able to manage challenging behaviour and create/review action plans
  • Ability to handle difficult situations and conversations while maintaining professional standards

Working Conditions

  • Travel may be required.
  • Must be able to lift up to 30 pounds at times.
  • Ability to set up and participate in activities, which may include squatting, bending, and lifting.
  • Prolonged periods of standing or physical activity
  • Extensive direct interactions with clients
