Job description

Quality and Patient Safety Lead

Reporting to the Health Director, the Quality and Patient Safety Lead will provide leadership within Eskasoni Health’s overall health programs for quality, safety, accreditation, risk management; and responsibility to implement all aspects of comprehensive quality improvement, Patient Safety, patient family engagement, risk management and accreditation plans to achieve the best outcomes in health care program and service delivery, safe care and continuous improvement to meet or exceed all accreditation requirements.

The lead actively seeks and implements solutions related to system issues for Eskasoni Health’s programs and services while meeting the goals and objectives of Eskasoni Health. The position:

  • provides leadership, advice and guidance, and support to the Health Director, program leads, staff and physicians on systems work redesign and supports increased performance in alignment with Eskasoni’s program/service goals, objectives and standards,
  • provides information, and guidance regarding evidence informed best practices and Patient Safety;
  • leads and provides expertise in the development of effective care delivery models which are responsive to current and future health human resources and/or health care trends;
  • leads and directs implementation of evidence based practice that improves patient care outcomes and experience;
  • works collaboratively with staff, physicians and leaders to apply evidence-based findings to clinical practices;
  • and provides support, including but not limited to, literature reviews, environmental scans, best practice guidelines and legislation applicable to health care.

Quality and Patient Safety Leaders hold many responsibilities including leading investigations of Patient Safety incidents, facilitating quality reviews, trending analysis, leading and/or providing improvement coaching for quality improvement initiatives, supporting accreditation activities, as well as providing education and training related to Quality and Patient Safety related priorities, policies and procedures.

Requirements

  • Undergraduate degree from a health care field required.
  • Minimum of two years Quality and Patient Safety Leadership experience, which includes two years of relevant clinical experience in a health care environment including expertise to critically review health records as part of the quality review process
  • Demonstrated experience and commitment to Patient Safety, quality improvement, risk management, infection prevention and control and education in a community health environment.
  • Demonstrated experience and expertise in developing and maintaining strategic partnerships
  • Certificate or training related to quality improvement, Patient Safety, risk management, or related program an asset
  • Experience in planning and implementing a variety of strategic activities related to accreditation, safety, quality improvement, infection control and prevention practices, risk management, patient relations and Patient engagement, while ensuring appropriate confidentiality
  • Experience engaging teams, leaders, medical staff, clinicians, patients/clients and stakeholders in a collaborative, participatory manner
  • Experience and demonstrated expertise within data management, data analysis and statistical manipulation; excellent report writing and presentation skills, presenting results in a meaningful manner
  • Excellent knowledge of word processing, spreadsheet, database, presentation and graphics software required
  • Experience in project management and program evaluation an asset.
  • Demonstrated ability of independent decision making, creativity, self-direction, motivation, change management and team building/ leadership skills
  • Excellent organizational skills and ability to meet deadlines in a fast- paced environment
  • Demonstrates a positive, solution oriented and collaborative approach and is comfortable navigating complex system issues, and implementing innovation solutions to problems

Term: Full time (37.5 hours per week)
