Job description

Safety at Home Coordinator

Community Support Services of Niagara (CSSN) is currently seeking a part-time permanent (25 hours per week) Safety at Home Coordinator to add to our team. CSSN is a volunteer based, not-for-profit agency providing comprehensive support services to assist seniors and adults with disabilities to live independently in their homes. Reporting to the Program Manager, the successful candidate will be responsible for completing in-home fall risk assessments for our Let’s Go Home (LEGHO) program.

Responsibilities:

  • Completes client intakes and in-home risk assessments.
  • Develops a service plan with the client/client’s family for the implementation of recommendations to reduce risk of falls e.g. installation of safety devices, repairs, changes in home environment.
  • Assists client/client’s family to access assistive devices and makes internal and external referrals as needed and appropriate.
  • Follows up with the family to determine success of service plan.
  • Works collaboratively with the LEGHO Navigators to ensure recommendations are implemented and clients/client’s family is connected to appropriate services.
  • Maintains regular contact with clients to ensure satisfaction with program.
  • Maintains accurate hardcopy and electronic client files.
  • Maintains confidentiality of client/agency information
  • Participates as a CSSN team member, promoting the agency and contributing to quality improvements.
  • Attends meetings and training, as required.
  • Maintains current knowledge of community resources and services.
  • Performs other duties as assigned.

Qualifications and Requirements:

  • Degree/diploma in social services or a relevant health care field
  • Previous experience in client assessments
  • Knowledge of senior’s issues, healthcare system and community resources
  • Excellent interpersonal and conflict resolution skills
  • Excellent written and oral communication skills and attention to detail
  • Excellent organizational skills
  • Ability to take initiative, work independently and as a team
  • Proficient computer skills – Microsoft Office, database management
  • Valid Ontario driver’s license and use of automobile is required.
  • Bilingual in French and English is preferred
