Position Description

Reporting to the Justice Program Manager, the Social and Cultural Wellness Worker shall deliver culturally safe, trauma informed, clinical case management mental health and addictions services within correctional facilities in Ontario. This position includes counselling with individuals and therapeutic groups, linking individuals to appropriate services and maintaining accurate client records. This program will include the development and delivery of strength-based programming, and aftercare through relationship building with local service providers to be able to assist and support individuals in making a greater positive impact within the community. The Wellness Worker will work within the Social and Cultural Wellness program to help deliver and improve the services offered to the relevant communities.

Reports to: Justice Programs Manager

Responsibilities

Case Management

Ø Provide intake screening, which includes collecting referral information and forms, inputting into Mustimuhw cEMR (MIS) and consulting with a Program Manager around the need for a crisis consult and staff assignment

Ø Develop an Initial Intake/Readiness Assessment at first contact and create an individualized Care Plan within 30 days for each client

Ø Provide counselling, information, education, and guidance to clients regarding mental health, addictions and life stabilization skills

Ø Provide counseling to clients on specific ways to recognize and control problem behaviours, and information, education, and support to clients to manage their own behaviour

Ø Monitor clients' mental status and behavior for decline and take appropriate action

Ø If requested by client, facilitate traditional healing services. Note: This could be an alternative to, or in conjunction with contemporary methods

Ø Coordinate and facilitate individual and/or group counselling to clients. This could include sharing circles, therapeutic debriefing sessions, etc

Ø Participate in inter-disciplinary case conferences

Ø Keep accurate, timely client contact records in accordance with program requirements

Ø All data entry to Mustimuhw cEMR (MIS) should be completed on a weekly basis.

Ø Assist and respond to crisis and post-crisis situations as required and complete individualized safety plans

Ø Alert the Program Manager and/or Director of Services on all critical cases and situations

Ø Other duties as assigned

External Referrals & Support

Ø Facilitate linkages to needed services within the broader service community for example: treatment centers, safety/stabilization beds

Ø Develop networks and partnerships with community resources

Ø Encourage and support clients in building their own social lives, independence, and support systems to reduce dependency on the Wellness Worker

Ø Advocate for client and coordinate case management meetings with relevant partners

Ø Facilitate access to Anishinaabe knowledge, traditions and Cultural beliefs in the healing process (if appropriate)

Ø Facilitate referrals to OTN for consults and assessments

Community Liaison

Ø Develop positive and good working relationships with community leaders, relevant professionals and parallel agencies to facilitate complete service to our clients

Ø Participate in community partnership meetings and relevant regional committees

Monitoring & Evaluation

Ø Maintain statistics required by the Ministry and the Program with regard to qualitative and quantitative aspects of the Program

Ø Work alongside Support Services to monitor that staff input data on a consistent basis, ensuring on-going quality service to clients, and for accurate reporting to ministry and communities

Ø Support the growth of the Service by having input on the development of program evaluation, design and visioning

Qualification

Ø University degree or college diploma from a recognized post-secondary institution (Psychology or Social Work)

Ø Excellent skills in the following areas: computer, organization, networking, interpersonal, presentation and communication (written and oral)

Ø Ability to work independently and in a team setting

Ø Experience in counselling adults in all areas of mental health and addictions

Ø Experience working in correctional or justice setting

Ø Ability to deal with crisis and post-crisis situations

Ø Experience working with and knowledge of Indian Residential School survivors

Ø Experience with Mustimuhw cEMR (MIS) considered an asset

Ø Knowledge of the Anishinaabe culture and language and experience in working in an Indigenous community or organization considered an asset

Position Requirements

Ø Valid driver’s license and vehicle

Ø Must be willing to travel when required
