Job description

60 hours bi-weekly, primarily days.

QUALIFICATIONS

Education and Certification

1. Diploma in Social Service Worker

2. Member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)

Experience

1. At least two (2) year of experience working as a Social Worker in a long-term care home or a setting that serves older adults.

Knowledge

1) A sound knowledge and background in geriatric, rehabilitation or long-term care social work

2) Knowledge of the requirements of the Long-Term Care Homes Act and corresponding regulations and other pertinent Federal, Provincial and Municipal statutes which govern or influence the operation of the home.

3) Demonstrated skills in the planning and coordination of a comprehensive Social Services program.

PERFORMANCE REQUIREMENTS

1. Provide strong, consistent leadership to ensure the maintenance of a cohesive and cooperative management team.

2. Ensure that effective management systems, including reporting and internal control, are maintained to ensure compliance with all applicable legislation, the Long-Term Care Homes Act and all OMNI Healthcare standards, policies and procedures.

3. Provide Clinical Social Service Work to residents for issues including (but not limited to) ethical dilemmas, abuse and neglect, situations of risk, complex family dynamics, mental health, addictions, low income, human rights, and consent and capacity

4. Establishing therapeutic relationships with residents and families

5. Ensuring that each Residents’ self-determination is safe guarded through actively involving them in planning and decision making.

6. Conducts a variety of cognitive assessments as needed (MOCA, MMSE)

7. Working with the Resident, Family and the multidisciplinary team proactively to solve interpersonal or behavioural issues a resident may be experiencing

8. Locating and arranging resources. Has the ability to understand how systems interact and what resources are available for the resident.

9. Mediating concerns that may arise between the resident and family or care team.

10. As a multidisciplinary team member, participates in ongoing care planning, involving the resident and family at all times.

11. Provides therapeutic services through individual and group counseling for residents and families, documents in the clinical record and evaluates effectiveness of these programs.

12. Provide therapeutic counselling in complex areas such as, life transitions, grief and loss as well as mental, physical and cognitive health

13. Works to coordinate services with the Psychogeriatric Geriatric Outreach Team specific to each Resident’s unique needs if/when needed.

14. Coordinate regular and relevant education for our residents, family and staff on topics related to long-term care such as dementia, grief/loss, resident bill of rights, zero tolerance of abuse and neglect, caregiver stress and resident-centered care,

15. Document interventions and recommendations in the resident health record, including care plan when relevant

16. Provide support to residents and families through referrals to community resources

17. Demonstrate and promote a philosophy which reflects and promotes the OMNI mission, vision and values.

18. Liaise with all other disciplines to coordinate and efficient multidisciplinary system of admission, transfer and discharge which effectively meets the needs of residents, family members, staff and the community.

19. Coordinate the admission interview to introduce the resident to the home and assess his/her future needs and preferences.

20. Ensure that documentation in the clinical record is accurate, current and complete.

21. Assist in the facilitation of processes required to ensure that MDS assessments and RAPs are an accurate reflection of the residents and the care that they are provided in the home.

22. Utilize Quality Matters tools in order to effectively evaluate and ensure continuous quality improvement of all aspects of care and services.

23. Utilize risk management tools which effectively identify and support the management of actual and potential risks.

24. Represent support and enthusiasm for each of the OMNI core programs.

25. Facilitate effective and supportive relationships with representatives of the Ministry of Long-Term Care, Ontario Health, Home and Community Care and The Canadian Council on Health Services Accreditation, community partners and other external agencies and organizations.

26. Participate in opportunities for professional development.

27. Perform other related duties as required.
