Job Description:
We are seeking a dedicated and compassionate Social Service Worker/Admissions Coordinator to join our team. In this role, you will be responsible for facilitating admissions, assessments, and providing social work support for our residents. This position requires strong interpersonal skills, empathy, and a commitment to improving the lives of those in our care.
Responsibilities:
- Assists the Director of Nursing/Admissions Manager with the admission of residents to the facility by:
- coordinating and managing the facility wait list
- initiating contact with the prospective resident/substitute decision maker
- scheduling and conducting pre-admission facility tours, in the absence of Admissions Manager
- Assists and supports new resident/substitute decision maker with admission into the facility. Provide counselling where necessary and may refer to external resource for further counselling.
- As a member of the health care team, participates in the multi- disciplinary process. Attends and schedules multidisciplinary care conference and completes required reports.
- Liaises with Community Care Access Centre representatives, hospital discharge planner, and other social workers in the community.
- Facilitates internal resident transfers in collaboration with the resident/substitute decision maker, facility staff, and other residents.
- Coordinates Ethics/Palliative Care Sub-Committee meetings and actively participates in facility committees.
- Completes required assessments on admission, including referral assessments by nursing and other disciplines and makes recommendations to better meet the resident care needs.
- Conduct special assessments based on identified needs, including recommendation and coordination of required services for residents with unusual, responsive and or unmanageable behaviours in collaboration with resident, family and interdisciplinary team
- Coordinates special group meetings with external services providers such as BSO, Geriatric Outreach program, etc.
- Participate and provide the necessary support and assistance for palliative care residents and their families
- Work closely in setting up PGT Program for residents needing the service, including follow-up as may be needed
- Ensure each resident has a current Health Card # and other related information that is updated, as needed
- Complete rate reductions for newly admitted residents, and annually thereafter - Ministry Portal
- Prepare and provide scheduled CQI reports to the Programs Director/Administrator. Actively participate in annual program evaluation for social work services, develop goals and objectives to improve the overall program, including the target dates to achieve desired results.
- Perform any other related duties as assigned.
Qualifications:
- Required - A College diploma or degree in Social Work from a recognized program. Must be registered under the Social Work and Social Services Work Act, 1998.
Must have at least two years relevant experience in social work or other related field.
Knowledge and Experience- Must possess specialized knowledge in gerontology or any field related to seniors. Experience in a long-term care setting would be preferable.
