Social Worker

Reporting to the Director, of Care the Social Worker will provide professional social work services to our 128 long term care residents. This will include: pre-admission data collection; admission services such as tours, interviewing the resident and family creating the Point Click Care record , liaison with the Continuing Care Division, physicians and community/hospital workers as necessary; provides supports the resident and family during the process; provides support during the settling in process; advocates on behalf of the resident with on issues related to psycho-social, religious or issues related to services provided by the Public Trustee and the Bloom Group Services; educates residents, families and care team members regarding advance care directives and health care decision making; assists in medication and conflict resolution issues as they arise; prepares and maintains statistical data and resident records

Qualifications: Master’s degree in Social Worker from a recognized university including or supplemented by courses in Gerontology; three (3) years recent related experience; registration with the BC College of Social Workers. The successful candidate will possess strong verbal and written communication skills; proven effective interpersonal and counselling skills; ability work effectively within a multidisciplinary care team; proven organizational skills; ability to set goals, prioritize work and adapt to changing priorities; demonstrated problem-solving approach; strong computer skills; and the physical ability to carry out the duties of the position.

This position is at Grade 2 (Grid 12 41.01 to $51.07) of the Health Science Professionals Provincial Agreement represented by HSA at St. Michael’s Centre

Job Type: Part-time

Salary: $41.01-$51.07 per hour
