Position Overview
Crosstown Family Health Team (CFHT) provides comprehensive primary health care, including chronic disease management, health promotion, disease prevention and mental health and wellness services to a rostered population of patients in its catchment area. CFHT employs an inter-professional team of health care providers who work in a collaborative model of care.
The Social Worker (SW) is critical to the provision of mental health services and the effective functioning of the FHT. This position requires strong interpersonal skills and an ability to work in a fast-paced, changing environment with patients and inter-professional health care providers. This role will require an understanding of mental health and addiction issues in primary care settings. This requires strong assessment and treatment skills to support patients dealing with challenges such as chronic health conditions, anxiety or mood disorders, family systems problems, economic stressors and other issues on the personal or community level. This is a contract position covering a parental leave.
Roles and Responsibilities
- Accept patients referred by the physicians or the interprofessional team
- Triage referrals and assign priority
- Provide psychosocial assessment and develop treatment plans for patients in collaboration with the Crosstown Family Health Team health professionals
- Refer patients or request consults when needed and assist the physicians and health providers with referrals to secondary and tertiary health care centres
- Assist patients in navigating services and connecting patients to those services
- Patient education and support, including mental health as it relates to prevention and chronic disease management including diabetes, COPD and seniors health
- Collaborate with team members to coordinate patient care
- Ability to conduct 5 individual sessions per day
- Experience facilitating clinical and psycho-educational groups, in person and virtually, independently and as part of a team
- Provide timely written documentation in the EMR using professional standards
Organizational:
- Collection of data as required for statistical/reporting purposes
- Preparation of monthly reports for the Ministry of Health and Long-Term Care
- Participate in team building, program development, and improvement activities
- Participate in staff meetings to exchange ideas, receive mutual support and monitor program outcomes
- Identify gaps and improvements in patient care and services
- Comply with established personnel policies, procedures, standards and guidelines
Other:
- Participate in established performance evaluation process and professional development goals
- Some assistance with clerical duties may be necessary, such as scheduling appointments
- Perform other related duties as assigned by the Executive Director or Lead Physician
Qualifications and Requirements
- Masters of Social Work, with 3-5 years experience in assessment and counseling/ therapy in primary care/ Family Health Team setting is preferred
- Registration in good standing with the Ontario College of Social Workers and Social Service Workers
- Experience in delivering therapeutic approaches such as Cognitive Behaviour Therapy, Solution Focused Brief Therapy, Trauma Informed Therapy, Mindfulness Based Therapy and Self Compassion Approaches
- Experperienced in suicidal risk assessment, co-developing safety plans, crisis intervention and verbal de-escalation skills
- Working knowledge of the Mental Health Act, Health Care Consent Act, Substitute Decisions Act & Community Mental Health Resources
- Demonstrated interpersonal skills including engagement, enthusiasm and passion
- Experience with families and adolescents an asset
- Additional training or credentialing is an asset (such as a Certified Addictions Counselor)
- Skills in program planning, implementation and evaluation
- Proficiency in computer applications: GSuite, GMail, Google Docs and Sheets, Zoom, Survey Monkey, or similar programs
- Demonstrated planning, problem solving, written and oral communication, and conflict resolution skills
- Experience working with diverse population and promoting inclusion
- Intercultural competency skills
- Ability to maintain high standards of professionalism and confidentiality
- Ability to work independently as part of a collaborative team
- Ability to learn and work with electronic medical records
- Willingness to engage in ongoing professional development and clinical training
- Must be able to work flexible hours, including evenings (to 8pm)
- Must be able to work from home having experience in conducting virtual counseling (video conferencing or over the phone)
We offer salary commensurate with experience between $36 - $38 per hour.
We are currently looking for someone who can work in a hybrid model; from home and in office.
We offer a set work schedule, and HOOPP. We have a high demand for SW services and are looking for someone to jump in. We plan to have overlap for training purposes.
