The stated hours of work are approximate as schedules are subject to change with the operational needs of the Health Centre. Changes made to hours of work will follow the terms of the applicable collective agreement, company policy and/or legislation.

WHAT YOU’LL DO:

Reporting to the Manager for the Crisis Response Unit (CRU) and CRU Short Stay unit (CRU SS), the Social Worker will be responsible for the provision and coordination of mental health intervention, assessment, navigation, and coordination duties for individuals presenting to the CRU and those admitted to the CRU Short Stay Unit. This includes 72-hour post discharge calls to ensure system supports have been activated and coordinate further services as required. Provision of services will be done through direct service interactions as well as in consultation and collaboration with hospital and community team members, with an underlying foundation on patient and family centered care.

WHAT YOU’LL BRING:

·Master of Social Work or related social service field.

·Minimum of 2 years’ experience providing crisis intervention services including assessment, intervention, and system navigation.

·Registration with the Ontario College of Social Workers and Social Service Workers or the College of Registered Psychotherapists of Ontario.

·Applied Suicide Intervention Skills Training preferred

WHO YOU ARE:

·Knowledge of emergency crisis intervention approaches, therapeutic interventions, community resource activation.

·Experience and working knowledge in the treatment of concurrent disorders, dual diagnosis, and a well-established understanding of trauma informed practice.

·Excellentorganizational skills, communication and interpersonal skills, and the ability to manage competing and changing priorities.

·Current knowledge of the Mental Health Act and Health Care Consent Act.

·Well established ability to work on an interdisciplinary team and offer input into team approaches to intervention(s).

·Established crisis intervention and de-escalation skills.

·Experience working in an emergency department with individuals in mental health crisis.

·Experience working in collaboration with first responders in intervening with individuals in mental health crisis.
