Rapids Family Health Team is seeking an enthusiastic, professional Social Worker, MSW or Psychotherapist with excellent communication and interpersonal skills to work collaboratively with other health care professionals to manage the needs of patients. This is a mobile position; travel may be required between Rapids Family Health Team sites as well as throughout the community based on client / community needs and business requirements.
Working in partnership with Rapids Family Health Team professionals to perform patient psychosocial assessments, provide short-term or single session counseling, crisis intervention, education/advocacy, facilitating psycho educational groups and case management. Performing these roles well will reduce unnecessary office visits and visits to after-care clinics or emergency rooms.
Education and Qualifications:
- Masters of Social Work or Masters of Psychotherapy degree from a recognized University
- Current membership and in good standing with the Ontario College of Social Workers and Social Service Workers or College of Registered Psychotherapists of Ontario
- Working knowledge of the Mental Health Act, Healthcare Consent Act, Substitute Decisions Act, other relevant legislation as well as community mental health and addictions resources
- Experience working as a Social Worker, MSW or Psychotherapist working with patients and families providing counseling / psychotherapy
- Demonstrated knowledge and experience with a variety of crisis assessment and intervention techniques and support strategies for mental health and/or addictions
- Efficient computer skills, working knowledge of electronic medical records
- Experience in a multidisciplinary setting will be considered an asset
This is an ONA Union position; compensation is governed as per the current Collective Agreement.
