Position Summary:
  • In consultation and collaboration with Electrophysiologists, Cardiologists and other providers, assist in the triage of patients for Pacemaker/ICD/CRT Implantation, Arrhythmia assessment and intervention and follow up care within Trillium Health Partners - Mississauga Hospital and between referring centres to facilitate timely access to services.
  • Wait list management including notifying physicians of changes in patients condition while on wait list
  • Data entry, management and report writing of the Cardiac Care Network of Ontario WTIS-CCN application - ICD/CRT and arrhythmia management and maintenance and dept. database(s).
  • Outcomes data collection, entry and management of data base
  • Assists with quality assurance activities and prepare and/or help coordinate monthly statistics
  • Establishing monitoring and reporting systems for quality assurance.
  • Coordination with other Arrhythmia Service Triage and CCN coordinators as a LHIN Regional Centre
  • Coordinate industry representatives for clinical support
  • Manage anticoagulation and cardiac medications pre-operative with physicians
  • Knowledge of best practices for patients referred to Arrhythmia Service
  • Performs other duties consistent with the role as required.
  • Support cardiac triage offices as needed.
Key Qualifications:

THP is committed to internal talent, and therefore priority will be given to internal applicants.
  • Current registration with CNO is required
  • Completion of Critical Care Certificate or CCNc required
  • 3-5 years Cardiac Critical Care experience required with comprehensive knowledge of arrhythmia management and device therapy and technology required.
  • Baccalaureate Degree in Nursing, Master's Degree Preferred
  • Clinical knowledge of results of cardiac diagnostic testing: 12 lead ECG interpretation, stress test, echo, perfusion scans, and coronary angiogram.
  • Excellent communication and interpersonal skills required
  • Ability to work collaboratively with cardiac cath and cardiac surgery triage offices.
  • Ability to represent Trillium Health Partners and Cardiac Health program in a positive way to our patients, families, referring physicians and hospitals
  • Ability to think critically, triage effectively and prioritize patient needs
  • Highly effective decision making, problem solving and organization skills
  • Ability to work collaboratively with the broader team members including the Regional referring centers, Cardiologists, referring physicians and their offices, inpatient units, Cardiac Care Network
  • Work within the mandate of Cardiac Care Network of Ontario (CCN) including statistical data collection, reporting within set time frames, report generation, statistical needs as required by the program.
  • Good working knowledge of a computer including programs such as Excel, Microsoft Word (testing required)
  • Able to function independently
  • Positive attitude, enthusiasm, self-starter
  • Positive attendance and work record essential
