Home Horizon Georgian Triangle Residential Services is a not-for-profit registered charity dedicated to transform the lives of youth at risk of homelessness by providing them with the opportunity to rebuild their lives and achieve their full potential. Youth are in the process of transitioning into adulthood, and due to challenging circumstances, may not yet have acquired the health, personal, social, and life skills that make independent living possible.

Youth Outreach Worker Job Description

This position provides outreach services through the Youth Outreach program with the goal of connecting and engaging with youth at risk of, or experiencing homelessness. Taking direction from the Youth Outreach Program Coordinator and ultimately reporting to the Executive Director, the Outreach Worker is responsible for providing services, identifying needs and making appropriate referrals for services and supports within the community, while also providing peer support and basic needs and supplies.

Community Relations

· Respond to phone inquiries and referrals from youth, families, service providers and the community at large.

Youth Outreach and Engagement

· Engage with youth who are precariously housed to build trust and connection.

· Provide positive interactions, crisis intervention, peer support and practical assistance to youth who access the program.

· Ensure respectful dialogue that embraces the dignity and worth of each young person;

· Reflect the core values of Home Horizon by adhering to an anti-oppressive stance;

· Use social media platforms to reach vulnerable youth who still have access to technology but are living unsafe with family, friends, opportunistic strangers or predators;

Case Management

· Keep a “warm hold” on youth on our wait list to ensure that they continue to receive counselling, support, advocacy, referrals and hope;

· Assist youth in navigating relevant systems to ensure they access the right services for and advocate on their behalf as needed;

· Accompanying youth to appointments if desired;

· Engage youth in Home Horizon activities where appropriate;

· Develop case plans for youth including referrals to all desired services;

· Maintain accurate records and statistics related to the activities undertaken and outcomes achieved, providing quarterly reports;

· Update relevant databases including Coordinated Access and HIFIS.

· Attend any necessary trainings and seminars, maintaining an up-to-date knowledge regarding issues and trends related to youth homelessness;

· Identify and make appropriate referrals for services and supports to outside agencies related to mental health, addictions, medical, financial, legal, housing, educational, employment, etc.

· Deliver/provide materials and supplies (clothing, gift cards, hygiene products, etc.) to youth in need throughout the community

Supporting Youth in the House

· Assist internal clients in the transition from Home Horizon to independent living (house/apartment searches, employment, budgeting, etc.)

Administration

· Report directly to the Executive Director.

· Attend staff meetings as requested.

· Record all expenses as per Home Horizon policies

Job Types: Full-time, Part-time

Salary: From $20.75 per hour

Expected hours: 28 per week
