Job description

The Adams Lake Indian Band (ALIB), belongs to the Secwepemc Nation and is a member of the Shuswap Nation Tribal Council (SNTC), which consists of 10 Secwepemc Bands. Over the past decades, ALIB has grown tremendously, which has driven the charge for significant changes within the community. They have developed new tools to deliver services in a changing world, which has resulted in more and varied opportunities to create and advance band programs, develop economic initiatives and to provide meaningful employment to more families. The various departments work as a collective team; dedicated to providing successful programs while carrying out their roles and responsibilities. They strive to create an atmosphere of trust and respect that honors the Secwepemc culture.

Adams Lake Indian Band is seeking one fulltime Case Manager, reporting to the Clinical Health Director. The Case Manager at Adams Lake Indian Band, you will play a pivotal role in coordinating complex and concurrent health care needs, including those who struggle with mental health and addictions, chronic multiple illness, accommodation requirements, and intergenerational trauma related matters. The primary responsibility of this role is to serve as the key point of contact, providing support and facilitating access to necessary resources, with a focus on cultural sensitivity to multiple stakeholders.

DUTIES INCLUDE:

  • Performs all duties and responsibilities in accordance with Adams Lake Indian Band policies and procedures
  • Concurrent Disorders Focus: Coordinate wrap around care and health services from multiple providers to ensure holistic support. Care coordination could include diabetes nurse, NP, Psychiatry, Home and Community Care, Mental Health Therapy, Lab services, and other specialized services.
  • Client Coordination: Act as a liaison between community members and health care providers, advocating for clients with complex care needs, including mental health and addiction challenges.
  • Assessment and Planning: Conduct thorough assessments of clients' health care requirements, paying special attention to mental health and addictions, and collaborate with relevant stakeholders to develop individualized care plans.
  • Cultural Sensitivity: Integrate cultural competence into case management practices, ensuring that services align with the cultural values and preferences of the community, especially in the context of colonial traumas.
  • Resource Navigation: Serve as a central point of access for clients, guiding them through available resources, programs, and services to meet their health care needs, including mental health and addiction support.
  • Collaboration: Establish and maintain strong working relationships with healthcare professionals, community agencies, and other service providers to ensure a holistic and integrated approach to mental health and addiction care.
  • Support: Work with ALIB Band Administration, 3rd parties and employees to support return to work, accommodations and sickness absence management as appropriate
  • Other related duties as required

REQUIRED QUALIFICATIONS, SKILLS AND ABILILITIES:

  • Bachelor's degree in social work, health care, or a related field.
  • 1-3 years previous experience in case management, specifically addressing mental health, addictions, and concurrent disorders, preferably within an Indigenous community context.
  • Knowledge and understanding of Aboriginal culture, traditions, and the impact of colonial traumas on mental health.
  • Excellent communication and interpersonal skills.
  • Strong organizational and problem-solving abilities.
  • Possess strong computer skills using MS Office software
  • Must have well developed organizational, verbal, and written communication skills
  • Ability to develop rapport
  • Ability to work in a team orientated environment
  • Must be flexible, adaptable, and able to work effectively in a variety of settings and in cross cultural situations

OCCUPATIONAL REQUIREMENTS (REQUIRED DOCUMENTS)

Current Criminal Record Check with Vulnerable sector clearance (original)

Valid B.C. Class 5 Driver's License and Abstract (copy required)

TERMS OF EMPLOYMENT:

Full time permanent position

Wage offered will be dependent on education and experience varying from $36 to $55 per hour.

Upon completion of probation ALIB offers comprehensive Benefits and a Pension Plan.

Hours per week: 75 hours biweekly (max 37.5 hrs./week), with flexibility in hours/days of work.

Position will remain open until a suitable candidate is available.
