Position Description
Kenora Chiefs Advisory is seeking a Child and Youth Development Services Support Worker to help create links between appropriate health and social resources for children and families in the 13 First Nation communities. They will assist clients to engage proactively with the community, family and children and assist individuals in maintaining resilient-focused wellness by offering a variety of services and supports
Position Title: Child and Youth Development Services Support Worker
Department: Development Services of Children and Youth
Term: Full time, Fixed Term till May 30,2024
Location: 240 Veterans Drive, 3rdFloor, Kenora, Ontario
Reports to: Reports to: NBDS Program Manager
Summary of Duties
- Develop an Initial Intake Assessment and create an individualized Care Plan and referrals
- Provide counselling, information, education and guidance to clients with regard to rehabilitation services
- Advocate and monitor clients’ need and take appropriate action
- Participate in inter-disciplinary case conferences
- Keep accurate, timely client contact records in accordance with program requirements
- Facilitate linkages to needed services within the broader service community
- Develop networks and partnerships with community resources
- Advocate for client and coordinate case management meetings with relevant partners
- Facilitate access to Anishinaabe knowledge, traditions and cultural beliefs in the healing process (if appropriate)
- Maintain statistics required by the Ministry and the Program with regard to qualitative and quantitative aspects of the Program
- Perform additional functions as required
Qualifications
- Ability to work independently and in a team setting
- Excellent skills in the following areas: computer, organization, networking, interpersonal, presentation and communication (written & oral)
- Experience working with children and families
- Knowledge of the Anishinaabe culture and language and experience in working in First Nation communities
- University degree or college diploma from a recognized post-secondary institution (social work)
Position Requirements
- Valid driver’s license and vehicle
- Must be willing to travel when required
