JOB SUMMARY
Reporting to the Director of Care, the Clinical Practice Leader is responsible for facilitating and providing leadership in the development of clinical skills, promoting best and professional practice, and initiating quality management activities and research at a Lakeridge Gardens 320 bed long term care home built on the Ajax-Pickering hospital site in the delivery of excellent care for residents.
Having opened to residents in early 2022, this new home is part of the government's plan to improve long-term care delivery and capacity in Ontario and in the Durham Region. This state-of-the-art long-term care home sets a new bar for seniors' care, with dining and outdoor space on every floor, a large courtyard, onsite diabetes care and other important innovative features.
DUTIES AND RESPONSIBILITIES:
The Clinical Practice Leader will:
" Develop, implement and promote best practices program for nursing to strengthen professional practice and promote an interdisciplinary team approach to resident care
" Design learning needs assessment and develop in-service and continuing education programs to support these needs and to keep staff updated with new practices and added skills in accordance with the Fixing Long Term Care Act, 2021.
" Support and initiate continuous quality improvement activities in nursing that support the overall CQI program as well as the Home's mission, vision and values
" Lead the department in the recruitment, selection and orientation of nursing staff and students
" Supervise all student placements and new hires while on probation
" Participate in research contributing to improved resident care and advances in nursing practice
" Effectively plan, both strategically and operationally, to address the changing and increasingly complex care needs of the residents
" Effectively communicate with nursing staff as well as the interdisciplinary team, families, residents and professional colleagues
" Be required to work occasional weekends and have flexibility to work all three shifts as needed
QUALIFICATIONS:
" Bachelor of Science in Nursing;
" Current registration as a Registered Nurse in good standing with the College of Nurses of Ontario;
" Master's Degree in Nursing or Adult Education is preferred; Minimum 3 to 5 years of experience in a similar role
" Experience at a management or front-line RN level in long term care for a minimum of two years
" Proficiency in RAI MDS quarterly coding and care plan development requirements for the LTC sector for a minimum of two years
" Completion of Basic Cardiac Life Support (B.C.L.S) program;
" Completion of BPSO Best Practice Champion certificate preferred;
" Theoretical and practical experience in adult education including educational program planning, development and evaluation;
" Knowledge of the Standards and Best Practices of the College of Nurses of Ontario, LTC Standards, Pharmacy Standards, Regulated Health Professionals Act, and all other applicable legislation
" Strong coaching, mentoring and leadership skills
" Excellent oral and written communication skills, including effective presentation and facilitation skills
" Must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months;

" Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area; and;
" Proven good attendance record with capability of maintaining this same standard;
