Position Overview

Reporting to the Chief of Nursing Practice and Clinical innovation, the Clinical Practice Leader is responsible for ensuring that client care services are delivered in accordance with Helping Hands Healthcare, Mission, Vision and Values. The Practice Leader will focus on training and supporting the competency development of nurses and personal support workers within the domains of home care, critical care, emergency, and general medicine and surgery.

The Practice Leader advances professional practice and clinical care excellence through the design and implementation of educational programs and clinical processes that enable staff to deliver safe and competent patient and family centered care. The Practice Leader collaborates with our hospital and community partners to ensure the ongoing provision of clinical excellence and expertise regarding best practices for community care, and hospital settings.

The role focus upon the identification of learning and practice development needs for our nursing and personal support staff. Innovation and participation in strategic design and process development will be an integral part of the role.

Responsibilities

  • Demonstrates a high level of understanding, knowledge and skill in both critical care and home care nursing.
  • Initiate, participate and contribute to the development of clinical knowledge and skills of new and existing nurses and personal support workers.Collaborate with our hospital clinical partners to provide ongoing clinical competency of our staff through site visits and consultation with staff.
  • In collaboration with the clinical team, address staff performance/clinical gaps and develop supportive education plans to address learning needs.
  • Contribute to the nursing and personal support competency evaluation, by participating in the strategic design of annual reviews.
  • Assess, evaluate, and prepare nursing care plans for home and community clients, including home safety assessments and joint visits.
  • In collaboration with the Chief Nurse, and the Human Resources (HR) Department, develop, modify, participate, and enhance the interview and orientation process for our nurses and personal support workers.
  • Develop and maintain clinical resources and orientation materials, reflective of changing best practices, Accreditation Canada standards and community requirements that contribute to an internal education library.
  • Participate in the identification and formulation of quality improvement initiatives, identifying issues and trends in Clinical Practice in collaboration with the Chief Nurse.
  • Update and maintain nursing and personal support policies and procedures.

Qualifications

  • Registered nurse (RN), or advanced practice nurse who is recognized as a clinical expert with a strong understanding of clinical care standards for Critical Care, and Community nursing.
  • Minimum 3 years of clinical experience in acute care, preferably including community experience.
  • Minimum 3 years of experience in teaching, working with learners or precepting.
  • Previous experience as a Clinical Instructor, nurse educator is an asset.
  • Experience designing, developing and delivering adult education in different learning environments.
  • Excellent communication and interpersonal skills demonstrated ability to work collaboratively and build and maintain strong relationships.
  • Demonstrated experience in change management principles and problem-solving skills.
  • Demonstrated ability to adopt quality improvement methods an asset.
  • CNA certification in a relevant nursing specialty area an asset.
  • Excellent time management and organizational skills, and ability to prioritize workload.
  • Demonstrated ability to develop a healthy workplace including fair and equitable work practices.
  • Valid Ontario Drivers License and access to a vehicle.

EDUCATION

  • RN with current Certificate of Competence issued by the College of Nurses of Ontario.
  • Undergraduate degree in Nursing.
  • Master’s in Nursing or Health Discipline or Education – Preferred or in progress.
  • BLS & ACLS Certificate required.
  • Adult Education or Critical Care Certification an asset.
