Community Support Services Worker
Full Time – 18 month contract -
Starting salary: $51,293.00
Reporting to the Community Support Services (CSS) Program Manager, the successful candidate will work within the CSS Team to provide a community and volunteer-based approach to service delivery to seniors, adults with physical disabilities and caregivers. The primary objective of the team is to assist clients to remain independent in their home for as long as they choose to. Internal Community Support Services (CSS) offered include: Meals on wheels, transportation and deliveries, grocery bus, friendly visiting, caregiver support, Telephone Assurance, crisis intervention & assistance, foot care, and a directory of workers for homemaking, home maintenance, respite care, and snow removal.
Description of Responsibilities
- Coordinate Community Support Services for seniors, adults with a physical disability and their caregivers.
- Assess clients to determine their needs and preferences for appropriate services, informing them of available internal and external programs and services, and making appropriate referrals.
- Ensure the accuracy and confidentiality of client files in the client information management system
- Conduct visits with clients as needed and keep up-to-date client records.
- Liaise with community partners to ensure effective provision of service delivery.
- Support program development, promotion & evaluation.
- Assist in the orientation of volunteers and students.
- Work within the CSS team and larger multi-service team of the Centre to accomplish diverse tasks.
- Perform required administrative functions.
Qualifications
- Post-secondary education in Social Sciences, Gerontology or a related field;
- Previous related experience in home Support Services;
- Experience working with seniors, adults with physical disabilities and caregivers;
- Excellent communication skills and strong interpersonal skills;
- Ability to work and function as a member of the Community Support Services team and the larger Resource Centre team;
- Ability to support volunteers;
- A commitment to high quality, client centered services through superior organizational and problem-solving skills.
- Superior computer competencies (Word, Excel, Outlook, CIMS, others)
- Oral and written fluency in English and French. French language skill is mandatory.
- Valid Driver’s License and access to a vehicle
- Excellent attention to detail;
- Excellent telephone manner/skills.
