QUALIFICATIONS


  • Grade 12 education (MB Standards)
  • Minimum one (1) year experience working with families facing multiple challenges
  • Province of Manitoba Class 5 Drivers Licence, and access to a personal vehicle to provide service within Prairie Mountain Health
  • Proficiency with Microsoft programs (Outlook, Word, Excel, Access and PowerPoint), as well as Internet applications and other Information Technology
  • Ability to manage time effectively
  • Ability to adapt to a work environment within an organization that embraces change
  • Demonstrated ability to work effectively as a team member
  • Demonstrated ability to establish and maintain positive interpersonal relationships

POSITION SUMMARY

Families First Home Visitors are Public Health Staff whose skill lies in building secure attachment relationships between parent and child. Families First Home Visitors work within their scope to help families reach their goals. When further assistance is needed, Families First Home Visitors support families to make self-referrals to appropriate staff or agencies. They communicate their observations and interventions to Public Health Nurses, who function as case managers for the family. The Families First Home Visitor receives program supervision from designated Public Health Nurse, Families First Coordinators.

RESPONSIBILITIES:

Overview:

  • Establishes and maintains trusting relationships with families.
  • Maintains appropriate boundaries within Families First Home Visitor role.
  • Schedules and conducts home visits with assigned families according to program guidelines.
  • Works flexible hours, including evenings and weekends, based on families’ needs.
  • Demonstrates initiative.
  • Participates in ongoing education and training including completion of Integrated Strategies and Tier Training as required.
  • Demonstrates problem-solving abilities that allow for the resolution of problems in an adaptable, flexible and ethical manner.
  • Demonstrates effective decision-making skills.
  • Demonstrates critical thinking skills.
  • Plans and facilitates approved curriculum and resources aimed at meeting the unique needs of each family.
  • Incorporates a strength-based, solution-focused approach that engages families in a process of change.
  • Works collaboratively with the Public Health team to achieve the goals of the Families First program:
    • To assist families to ensure the physical health and safety of their children.
    • To enable parents to enhance their parenting skills and to foster the development of a secure attachment with their children.
    • To support parents in their role of nurturing their children and providing appropriate physical, social and cognitive stimulation.
    • To facilitate the parents’ connections with community resources and to assist them in developing a sense of belonging.
  • Works closely with Public Health Nurse case managers to support families’ goals, optimal child development, and family functioning.
  • Works closely with Public Health Nurse, Families First Coordinators to engage in reflective practice and skill development.
  • Identifies and obtains support for complex family situations and uses standard curriculum emergently to address concerns.
  • Completes documentation and administrative tasks according to regional standards.
  • Maintains awareness of community resources and supports families to connect with appropriate resources.
  • Other duties as assigned.
