Intake Support Coordinator
Lakeview Recovery Community

Located in Gunn, Alberta, the Lakeview Recovery Community supports men living with addiction on their journey to a sustainable recovery. We deliver a full continuum of addiction recovery support in a community environment, focusing on healing and personal growth. Our goal is that individuals leave Lakeview with the life skills, tools, and resources to maintain long-term wellness and sustain recovery and having nurtured healthy relationships with friends and family.
Reporting to the Intake Manager the Intake Support Coordinator will assist the Intake Manager in coordinating the admission process for clients seeking treatment for substance use disorders. Working closely with clients, families, and the treatment team, the Intake Support Worker will provide administrative support, conduct intake assessments, and ensure a smooth transition into our program.

Role-specific Accountabilities

  • Assist the Intake Manager in coordinating the intake process, including scheduling appointments, conducting initial screenings and assessments, and verifying insurance coverage.
  • Greet clients and their families warmly upon arrival and provide them with information about our programs, services, and admission requirements.
  • Complete intake paperwork accurately and ensure all necessary consent forms, release of information agreements, and medical records are obtained and filed appropriately.
  • Conduct intake assessments to gather relevant information about clients' medical history, substance use patterns, treatment goals, and social support systems.
  • Complete risk management plans to ensure we can appropriately meet the needs of clients referred to us.
  • Collaborate with the Intake Manager, therapeutic and medical teams to determine the most appropriate level of care for each client based on their individual needs and clinical assessment findings.
  • Communicate effectively with clients, families, and referral sources to address questions, provide support, and facilitate the admissions process.
  • Maintain accurate and up-to-date records of client admissions, discharges, and demographic information in the electronic health record (EHR) system.
  • Provide ongoing support and guidance to clients and their families throughout the intake process, addressing any concerns or barriers to treatment with empathy and professionalism.
  • Actively promote the Recovery Community with internal and external stakeholders. Supporting the formulation of pathways into the Program.

Qualifications

  • Previous experience in a healthcare or behavioral health setting, particularly in admissions or intake coordination, is highly desirable.
  • Strong organizational skills and attention to detail, with the ability to manage multiple tasks simultaneously and prioritize workload effectively.
  • Excellent communication and interpersonal skills, with the ability to interact professionally with clients, families, and colleagues from diverse backgrounds.
  • Ability to maintain confidentiality and adhere to ethical standards in handling sensitive client information.
  • Compassionate and nonjudgmental attitude toward individuals seeking treatment for substance use disorders.
  • Successfully complete and pass a Vulnerable Persons Criminal Record Check prior to initiating employment and annually thereafter.
  • Clearance to be on premises by the Director of Security.
