Job Description: Reporting to the Primary Care Manager and in collaboration with the primary health care clinicians and team, this position supports effective relationships between the client and the health care team. The position works within the 3 pillars of the chronic disease management model to support client self management of their condition, support the provision of evidence based care as outlined in the care pathways for key chronic diseases and supports access to services that will positively influence the clients health.
Required Qualifications
Baccalaureate Degree in Nursing preferred
Current registration with the SRNA and licensed as a RN.
Knowledge of community needs assessment, community development and health promtion preferred.
Strong communication and public relations skills.
Three full time years' experience as a Registered Nurse working primarily with clients with chronic illness like Diabetes, HTN, COPD and CAD.
Valid driver's license, vehicle and ability to travel.
