The purpose of this position is to provide individualized support by enabling people to participate in their community, while their parent/caregiver is receiving respite. The Respite Support Worker’s role will be to provide information to the individual to make informed choice; provide opportunities to learn; and work with the individual and parent/caregiver to create a Person-Directed Plan, in support of the achievement of personal goals. This position will also assist individuals with any necessary medication administration and personal care support, as required.
Requirements/Qualifications:
- Ontario Secondary School Diploma or equivalent and a minimum 1-year experience in the Human Services field or Post Secondary School Diploma in a related field and a minimum of 6 months’ experience.
- Computer experience (Microsoft Office, Nucleus).
- Experience supporting people in community setting a definite asset.
- Good verbal and written communication skills.
- Up-to-date certificates in First Aid, C.P.R., C.P.I. and WHMIS.
- Valid Ontario Driver's License, Class G, with access to and use of a reliable vehicle and Third-Party Liability Insurance in the amount of two million dollars.
- Meet the requirements contained within Article 12.06 of the Collective Agreement.
- Possess an understanding of the Agency's Vision, Mission, Service and Guiding Principles.
Duties:
- Demonstrate a commitment to Community Living’s vision, mission, and values.
- Provide support for individuals to achieve goals and dreams, through the development of Individual Support Plans, while the caregiver/parent is receiving respite.
- Be aware of community resources, activities, and opportunities that promote the social inclusion of people supported.
- Support individuals to participate in the community; including educational, recreational, and religious activities.
- Assist individuals with personal care as required; including medication administration, skin care and personal hygiene.
- Assist with the development and maintenance of social and personal relationships.
- Provide information to individuals to support the development of informed decision-making skills.
- Facilitate the development of social networks for people, promoting the involvement of family, community groups, friends, employers, and others in the community.
- Complete computerized documentation, daily/weekly reporting, etc.
