RPN Part-Time - Hanover

We are currently seeking 1 part-time RPN's to join our wonderful team. This position is for our Intensive Community Supports Program where you will be working one on one with the supported individual in the client’s home in Hanover, ON, this position is for up to 24hrs/week. Non-Union Position.

What the position entails:

• Act as nurse, working with the family in providing exceptional person-centered supports
• Assist the family with the development and follow-through of Individual Support Plans
• Assess clients’ needs and encourage healthy life decisions
• Administering and documenting medications
• Monitoring the health and wellness of the client
• Facilitating communication between client and their medical support team with the assistance of the family
• Assisting with ordering and organizing medical support needs
• Collaborate with other services to provide dynamic and individualized community supports
• Assist with day-to-day needs
• Supporting the client with personal care needs
• Support physical and communication developments, or other goals set by the individuals

Qualifications:

• Currently registered and in good standing with Ontario College of Nurses
• Leadership experience considered an asset
• Demonstrated sound nursing and critical thinking skills
• Professional or personal experience working with individuals who have complex needs
• Ability to work independently and in collaboration with the family
• Valid CPR and First Aid certification
• A clean driver's abstract to be added to company insurance, if use of our vehicles is required
