Job description

Overview

This position is responsible for the provision of social work and support services to patients and families in the outpatient oncology clinic who are facing the impacts of potential or actual cancer diagnoses or changes in disease trajectory. This position is also responsible for leading the psychosocial oncology program at MGH.

Education

  • Member of the Ontario College of Social Workers & Social Service Workers or a candidate for
certification required.
  • Master of Social Work from an accredited School of Social Work, or equivalent degree as
recognized by the Ontario College of Social Workers & Social Service Workers required.
- de Souza - IPODE: Interprofessional Psychosocial Oncology course completed or a willingness to complete within 1 year
  • Managing Cancer and Living Meaningfully (CALM) workshop completion and certification, and a
willingness to bring CALM training to other MGH providers
  • de Souza, or equivalent, Interprofessional Education in Palliative and End of Life Care Course or a willingness to complete within 2 years

Qualifications

  • 1-3 years’ experience in oncology outpatient cares required.
  • Demonstrated ability to provide comprehensive assessment and treatment planning.
  • Demonstrated ability to advocate for services to meet complex needs of oncology/palliative patients.
-Demonstrated expertise and experience discussing end-of-life issues, advanced care planning etc. including MAID with patients, families and colleagues. - Demonstrated ability to provide educational and supportive counseling.
  • Demonstrated ability toprovide resource identification, referral and linking.
  • Demonstrated ability to provide collaboration and consultation with inter-professional team and community based service providers.
  • Demonstrated ability to work as a member of an interdisciplinary team and independently.
  • Demonstrated ability to partner with external oncology Social Workers and psychosocial oncology practitioners and program developers to form a community of practice and lead best practice in psychosocial oncology at MGH.
  • Participate in education to various groups including social work and other health professionals, students, patients and families, colleagues and community agencies.
  • Participate in professional, program and hospital development, planning, implementation and evaluation of service.
  • Strong written and verbal interpersonal and communication skills required.
  • Demonstrated ability to prioritize and work effectively in a fast paced environment required.
  • Demonstrated patient centered philosophy of care provision.
  • Demonstrated ability to welcome change and manage it innovatively.
  • Good work and attendance record required.
