ABOUT HOMES FOR HEROES FOUNDATION
H4HF is a transitional, supported independent living and affordable housing program for military Veterans in need of stable housing, offering 20 tiny homes in the Kingston Veterans Village to support residents through the assistance and advocacy of on-site Social Workers, with their goals to successfully re-enter mainstream society employed, stable and self-sufficient.

POSITION OBJECTIVE:
At the heart of our mission, the Case Worker, under the guidance of the Director of Operations, forms an integral part of our compassionate team at the Calgary H4HF Veteran Village. With genuine enthusiasm, this role is dedicated to delivering professional case management to our cherished Veteran residents. As a Social Worker, the focus extends beyond duty—it's about fostering skills, cultivating resources, and empowering independent living. Offering heartfelt counsel and unwavering advocacy, the goal is to facilitate access to a spectrum of supportive social services.
In this meaningful journey, our Case Worker also stands as a pillar of support, aligning with the principles of the provincial Residential Tenancy Act. They actively contribute to the seamless operation, and administration of the Veteran Village, working hand in hand with the entire team. Together, we create a haven where our Veterans thrive, and their well-being is at the forefront of everything we do.
DUTIES INCLUDE:
Case Management
· Conduct intake interviews to evaluate eligibility and make recommendations on potential residents.
· Conduct individualized needs assessments to identify resident needs, and develop individualized action plans to support residents, including programs that address financial security, legal aid, career retraining and employment, medical health, and other services, as well as social integration, and life skills in preparation for successful transition.
· Advocate and assist with accessing service providers and leveraging partnerships with Veterans Affairs Canada (VAC) and local community social service providers.
· Assess and support the resident’s current financial situation. Assist with applications for government income support, including for VAC funding and from other community partners.
· Schedule regular meetings with the resident to review their individualized action plan and assess identified goals, with opportunities to revise them as needed.
· Maintain reporting and case notes on residents’ progress. Complete final reporting, including questionnaires, surveys and exit interviews when a resident exits the H4HF program.
Property Management
· Responsible for adherence to the provincial Residential Tenancy Act (RTA), including administration related to moving in/out residents and execution of documentation.
· Conducting well-being and asset inspections.
Outreach & Partnerships
· Develop outreach relationships with the local housing and social services community.
· Assisting with Veteran Affairs Canada (VAC) applications.
· Develop a network of community resources partners.
· Seek alternative housing options for residents for long-term housing that may require supportive or alternative care.
KNOWLEDGE, SKILLS, ABILITIES
· In-depth knowledge of municipal and provincial social and government services, including experience working with Alberta Income Support (Alberta Works and AISH) and Calgary Homeless Foundation.
· Excellent interpersonal skills, including facilitation, conflict resolution and communication skills.
· Must have strong time management, problem solving and organizational skills; and possess the ability to perform duties independently in a high tempo environment.
· Knowledge and understanding of client service outcome tracking and experience with client assessment tools.
· Experience with the Veteran community is preferred.
QUALIFICATIONS
· Bachelor’s Degree in Social Work (or related Social Services field) with 2-3 years of experience in housing or a Diploma in Social Work with 3-5 years of experience in housing.
· Active registration with the Alberta College of Social Workers (ACSW).
· Proof of a Police Information Check.
· Proof of current Standard First Aid and CPR/AED (level C) certification.
Schedule: Monday-Friday 0900-1700h (40hr/ week)
