The Specialist - Palliative Care Implementation is responsible for project management support for the implementation of the Palliative Care Health Services Delivery Framework ("The Delivery Framework"), initially focused on model of care recommendations for adults in community-based settings in their OH Region. They are accountable for coordinating and tracking regional work to ensure timely completion of deliverables. They will support implementing organizations (e.g., LTCH, OHTs, FHTs, HSPs) to advance Palliative Care through quality improvement initiatives, to monitor risks and to sustain the spread and scale of successful initiatives. Where Indigenous communities wish to be involved, they will provide support to Indigenous communities and Indigenous organizations for their implementation efforts.
Here is what you will be doing:
  • Supports the work of the Lead – Palliative Care Implementation as well as the Regional Implementation team.
  • Evaluates, revises, and assists in the execution of plans to successfully deliver on accountabilities for a high-priority, complex initiative with multi-year duration.
  • Applies project management processes and tools.
  • Conducts monitoring and analysis to guide regional implementation of the Delivery Framework. Conducts regularly scheduled status reviews and escalates key issues/risks. Proposes and assists in the execution of solutions to address issues and mitigate risks.
  • Ensures implementation of data collection tools in implementing organizations to support monitoring and evaluation.
  • Utilizes data and evidence to help identify areas for clinical quality improvement. Additionally, supports implementing organizations to effectively use data to advance care, in collaboration with identified clinical leadership.
  • Drafts reports focused on implementation progress. Maintains documentation as required.
  • Expresses complex concepts effectively. Develops and presents material for review and informational purposes using excellent written, verbal, and graphical communications skills.
  • Works closely with regional and local clinical experts to organize Palliative Care education.
  • Works closely with regional and local clinical experts to organize Palliative Care coaching and mentoring.
  • Work closely with site leads, clinicians, and other stakeholders to identify opportunities to align clinical and operational priorities.
  • Develops and maintains regional and local relationships to advance regional priorities and initiative goals.
  • Supports the sharing of lessons learned and resources within their own and other OH regions.
Here is what you will need to be successful:
Education and Experience
  • A Bachelor’s degree is required, with a preference for a degree in health science, health administration, business administration or a related discipline.
  • A Master’s degree in one of the disciplines above is an asset.
  • Project management certification is an asset (i.e., PMP).
  • Three (3) years of project management experience is required, with a strong preference for project experience within the health care sector.
  • Experience with Palliative Care initiatives is an asset.
  • Experience with community health organizations or long-term care homes is an asset.
  • A strong understanding of regional priorities and local health care needs, within Ontario’s health care landscape.
Knowledge and Skills
  • A commitment to developing a coordinated and standardized approach for delivering Palliative Care services for all people in Ontario, regardless of their age or illness.
  • Knowledge of relevant legislation, including the Fixing Long-Term Care Homes Act, and the Excellent Care for All Act.
  • Strong planning and project management skills.
  • Ability to successfully execute large-scale, complex initiatives.
  • Excels in a fast-paced and objective-based environment.
  • Superior attention to detail with excellent organization and time management skills.
  • Able to prioritize, effectively anticipate and respond to issues as they arise.
  • Aptitude for proactive problem solving, by recommending thorough and practical solutions to a wide range of complex problems.
  • A strong understanding of the continuum of care, including clinical processes, with the ability to facilitate the effective use of clinical data for quality improvement purposes.
  • Excellent written and verbal communication skills.
  • Supports the advancement of equity, inclusion, and diversity across Ontario's health care system.
  • Commitment to engage with First Nations, Indigenous, Métis and urban Indigenous peoples as they self-determine their own approach to Palliative Care.
  • Advanced knowledge of Microsoft Outlook, Teams, Word, PowerPoint, Visio and Excel.
  • Fluency in French is an asset.
