Job Description: This person is responsible for assessment, design and implementation of Continuing Care services (home care, transition programs - respite, waiting placement, convalescence, palliative care), long term care, Geriatric Assessment and Rehabilitation Unit, Adult Day Program, and on-going case management services to assist clients to continue to live in the community. The Community Care Coordinator provides case management services to meet the client's needs in accordance with Saskatchewan Health Authority policies, procedures, and guidelines. Case Management is defined as: responsibility for an assigned caseload and accountability for all decisions and actions that impact individual cases.
Required Qualifications
Bachelor Degree/Diploma in Nursing or Bachelor Degree/Diploma in Psychiatric Nursing or Bachelor Degree in Social Work
Registered or eligible for registration with the appropriate licensing body
Experience
One year related experience in community health care or acute care.
