Job Description: In accordance with the mission, vision, and values of the Five Hills Health Region and Mental Health and Addictions services, reporting to the Executive Director Integrated Community Health Programs, the incumbent is accountable for assessment and coordination of initial service requests for all programs within Mental Health and Addictions Services in the Five Hills Health Region. The Intake worker provides screening, assessment, triage, coordination of case assignments, crisis response and short-term treatment services to clients presenting with a variety of mental health and addictions issues. The incumbent shall function within the practice principles of the Mental Health and Addictions Services' program management model.
Required Qualifications
Bachelor Degree in a health-related field
Registered or eligible for registration with the appropriate licensing body
The Saskatchewan Health Authority (SHA) requires a criminal record check (CRC) from every potential SHA team member. The CRC must be dated within the past six (6) months and be satisfactory to the SHA. A vulnerable sector check (VSC) is required for individuals providing patient/resident/client care to vulnerable persons.
