Home Instead® is looking for a dynamic and caring person to join our team as a Case Manager. An active LPN registration is an asset. The Case Manager works closely with families who are at the beginning of their care journey and are looking for care solutions for their loved ones. To be successful the candidate must:
- Be extremely articulate with Perfect Command of the English language.
- Have at least 2+ years of Home Care Experience
- Be able to work with a priority driven attitude
- Be able to multi task with a busy work pace
- Have confidence when communicating
- Be willing to follow a proven system
This position involves speaking with family members who are searching for answers to their senior loved ones care needs. You will interact with a variety of different people who are at different stages of their care journey,
You will need to use sales skills to explain to families why they should choose Home Instead for their loved ones. This role is responsible for effectively managing multiple demands that come with the changing needs of clients. This individual must be able to work in a fast-paced organization and possess excellent communication skills, be service-oriented and have the ability to work well in a team environment. Your health care background will help you understand the unique challenges our clients face. You will be busy and we are looking for that special person who loves hard work!
The Case Manager is expected to perform a variety of duties that relate to client care including care consultations with potential clients and family members, client/Care Giver introductions and quality assurance visits with existing clients. They use the consultative sales approach to determine each individual client’s needs to provide solutions and create a tailored service plan. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention as well as opportunities to increase service hours. The Case Manager will often speak with families who are unsure of which pathway is the “best” so our Coordinator must have a confident personality with the ability to communicate professionally.
This leadership position will include teaching, training our unregulated workforce. This will include classroom training, as well as in home visits to train and support our amazing Care Givers.
This is a full time position with 1-2 evenings per week and one in four weekends on call.
Qualifications
- Must have excellent written and verbal communication
- Direct experience working with Seniors and their families
- 5+ years experience in healthcare
- Experience with sales and customer service
- Reliable transportation
- LPN registration is an asset
- Supervisory experience is an asset
