Reports to: PPCS Manager
Reporting requirements: Basic Statistics – Daily/Weekly
Shift: Thursday – Monday: 12:00pm – 8:00pm
General Description
The Health Care Aide will provide personal care and support services for elderly, disabled, acute or chronically ill clients who require short-term assistance or ongoing support in maintaining personal independence in the home and community. The HCA will work closely with the Inii Healing House and Katoyiss clan.
Role and Responsibilities Duties
· Provides supportive/personal care to clients in activities of daily living under the direction of the PPCS Manager
· Creation and monitoring of client charting designated guidelines
· Perform vital signs, changing non-sterile dressings, testing blood sugar with a glucometer
· Maintains communication with Home Care/Community Health and relevant stakeholders regarding client care and emergency needs
· Ability to lift and transfer up to 100 kilograms
· Complete physical activities, e.g. lifting and transferring, bathing, monitoring the nutritional needs of clients
· Able to use tact and judgment in dealing with difficult situations, e.g. palliative care
· Able to work flexible hours to accommodate any client emergencies
· Client-centered, non-judgmental, and cognizant of client behaviors
· Mature, dependable, and strictly maintains confidentiality
· Interview clients, obtain history, prepare intake reports
· Submit complete referral forms as needed
· Follow up on client treatment plan
· Assist clients with organizing personal affairs as needed – under Supervisor's guidance
· Participate in weekly case consultations
· Participate in daily debriefing meetings
· Complete daily communications logs
· Ensure documentation is handled confidently and filed in client files as per file guidelines
· Assist in program delivery as needed
· Housekeeping duties per program guidelines: cooking and cleaning
· Confidentially offering transparent support – only high clearance access to client file notes/info (supervisor/director)
· Confidentially communicate with care providers/guardians and/or approved persons only.
Knowledge, Skills and Abilities
· Sound Knowledge and ability to demonstrate a practical application of the following techniques is considered an asset: crisis intervention, de-escalation skills, people management skills, conflict intervention and resolution
· Intermediate level of verbal and written communication and interpersonal skills
· Reliable, compassionate, and works collaboratively within a team environment.
· Organized, time-focused, and takes initiative
· Able to build and manage relationships based on trust and rapport
· Knowledge of Piikani/Blackfoot Culture and Language is an asset.
Qualification
· Health Care Aide certificate
· Mental Health Certifications or Training focused on Vulnerable sectors
· First Aide, CPR, Applied Suicide Intervention Skills Training (ASIST), Non-Violent Crisis Intervention (NVDI), Trauma-Informed Care, Security training would be considered an asset.
· Experience in mental health, counselling, care facility, with vulnerable populations in a client supervisory/ role.
· Hazardous Materials handling – Industrial cleaning certification an asset
· Comfortable in MS office suite
