Job Description: The Chronic Disease/Harm Reduction Educator reports to the Primary Health Care Manager. This position is responsible for the effective coordination of the Chronic Disease and Harm Reduction Programs to ensure high quality client care, through an effective and sustainable program delivery. The Chronic Disease/Harm Reduction Educator is responsible to work collaboratively with the Chronic Disease Nurse Educator, Primary Health Care Facilitator, Nurse Practitioners (RN/NP) and other staff and physicians to ensure delivery of an interdisciplinary approach. There may be some travel to areas within Integrated Rural Health.
Required Qualifications
Bachelor Degree in a health-related field
Registered or eligible for registration with the appropriate licensing body
Valid Class 5 driver's license
Knowledge, Skills and Abilities
Ability to communicate effectively both written and verbal
Ability to conduct client assessments and engage in effective treatment planning
Ability to initiate and exercise critical thinking and decision-making
Ability to prioritize, interpret and respond appropriately to issues or situations
Ability to provide public/educational presentations
Ability to travel extensively within designated area
Ability to work independently and as a team member
Problem solving skills
Proficient in computer software (Word, Power Point, Excel, Access, Outlook)

Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement. HSAS seniority within the Saskatchewan Health Authority will be considered
