Home Instead® is looking for a confident, charismatic and organized leader to join us in our mission to enhance the lives of aging adults and their families. The Client Care Coordinator is expected to perform a variety of duties that relate to Client Care including Care Consultations with potential clients and family members, client/Care Pro introductions and Quality Assurance visits with existing clients. This full time permanent position involves extensive one-on-one interaction with clients, caregivers and families in Richmond, North Delta and Central Surrey. Expanding market penetration through networking with key contacts is also an important aspect of the role.

The Client Care Coordinator is responsible for effectively managing multiple demands that come with the changing needs of clients. This individual must be able to work in a fast-paced organization and possess excellent communication and problem solving skills, be service-oriented and have the ability to work well in a team environment and independently. Having the ability to step in and take the lead with Client Care is an asset that allows continuity of care and inspiring moments for our Care Pro's. Our primary office is located centrally within Richmond and our service area includes Richmond, North Delta and Central Surrey. The Client Coordinator is required to have a valid license in order to support clients and Care Pros within our service area. Driving regularly within our service area is expected, as well as working from our office and opportunities to work remotely on occasion.

The Client Coordinator is expected to perform a variety of duties that relate to in home Client Care and service delivery for our clients and their families. They are expected to be responsible for providing the highest level of quality service and will use consultative skills to determine each client’s initial needs and provide solutions in the form of a tailored plan of care. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention.

Responsibilities

  • Ensure the needs of the clients, families, Care Pro's, and company are consistently being met
  • Meet with potential clients and family members to discuss their needs and provide solutions in the form of a plan of care
  • Have a growth mindset and sales acumen. Our priority is enhancing the lives of aging adults which requires a sales mindset when setting up plans of care
  • Create, manage and maintain client records in our operating system
  • Conduct client/Care Pro introductions with every new client
  • Develop and execute a Quality Assurance frequency schedule for each home care client
  • Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through Quality Assurance visits
  • Display confidence and expertise in providing high quality in home senior care
  • Exude confidence working within people's homes
  • Demonstrate open and effective communication with Clients, Managers, Colleagues and Care Pros
  • Other duties will be assigned as required

Requirements

  • 2 years experience in home care, senior care related industry or health care preferred, an equivalent combination of education and work experience may be considered
  • Experience as an RN, LPN, HCA or Social Worker preferred but not required
  • English fluency is required and additional languages are a bonus
  • Display a positive and healthy attitude in the field and in the office
  • Ability to work independently, maintain confidentiality of information and meet deadlines
  • Demonstrate consistent logic, rationality, objectivity and common sense in decision making while anticipating the consequences of decisions.
  • Proficiency with computers including Microsoft Office, (Outlook, Word, Excel, Teams), Zoom and the ability to learn new software (client database and scheduling)
  • Must be proficient at typing professional care plans, documents and emails with a minimum typing speed of 45 wpm
  • Generate new & creative approaches to problems or original modifications (innovations) to established approaches
  • Days of work will typically be during business hours from Monday - Friday but weekend and evening hours will be required occasionally as needed
  • Must possess a valid driver’s license and clean driving record
  • Must be comfortable driving regularly within Surrey, North Delta and Richmond
  • Must enjoy working within the community in peoples homes and community care settings
  • Must provide or complete a current criminal background check before being eligible to work
