Job Types: Full-time, Permanent

Salary: $60,000.00-$65,000.00 per year

Hours- Monday to Thursday 8:30 AM to 4:00 PM. Friday 8:30 AM to 2:30 PM. Rotational on-call.

Role Summary:

As a client Care Manager at IHAC you will lead a team committed to providing exceptional care for individuals facing behavioral issues and mental health challenges. Your role will involve creating a safe and nurturing environment where clients can thrive on their journey towards improved well-being and independence.

Key Responsibilities:

  • Develop and implement individualized care plans tailored to clients with complex behavioral issues and mental health concerns, ensuring their unique needs are addressed.
  • Leading the planning and execution of Person-Centered Meetings, fostering a collaborative approach to care that empowers clients to actively participate in their treatment and progress.
  • Oversea the consistent implementation of established procedures and interventions, maintaining a structure and supportive environment.
  • Support and develop programming to help individuals in care focus on positive behavior, mental wellness, and skill-building, enhancing client’s abilities to manage daily challenges.
  • Maintain accurate and up-to-date client records, documentation, and progress reports to ensure compliance with regulatory standards and facilitate effective communication among stakeholders.
  • Provide guidance, support, and coaching to your team members, promotion a cohesive and understanding approach to managing clients' unique needs.
  • Collaborate with healthcare professionals, therapists and external agencies to ensure a holistic and comprehensive approach to care for each client.
  • Help scheduling and coordinating efforts with scheduling to ensure support for clients at all times.
  • Participate in on-call rotations, offering guidance and crisis management support during off-hours as needed.

Qualifications

  • A Post-Secondary Degree in a related field of study in Human Services.
  • Clean Vulnerable Criminal Record Check.
  • 2+ years of relevant experience with strong understanding of behavioral challenges and mental health conditions.
  • Strong knowledge of person-centered and group program planning as well as best practices for residential support work.
  • Effective leadership skills, with the ability to mentor and guide a team in a dynamic environment.
  • Ability to navigate MS Office (including Excel, Word, Outlook and PowerPoint)
  • Well-developed written and verbal communication skills.
  • Demonstrated time management and able to meet tight deadlines.
  • Work well under pressure and in an ever-changing environment.
