Job Description: Supervises and coordinates staff. Assesses the needs of clients released from acute/respite care, develops care plans and coordinates delivery of services.
Required Qualifications
Baccalaureat degree in human services field
Experience
Previous: Thirty-six (36) months previous experience working in a community-based health care service environment.
Knowledge, Skills and Abilities
Ability to work independently
Basic computer skills
Communication skills
Interpersonal skills
Leadership skills
Organizational skills
Valid drivers license
