Summary

The Crisis Intervention and Assistance (CIA) Specialist provides urgent intervention and support in critical situations to eligible recipients with different types of needs until the situation is stabilized and a follow-up plan is in place. The target groups are vulnerable and at-risk seniors, persons with physical disabilities and/or their significant others. This includes people facing homelessness, a critical or impending change in life situation, abuse, or isolation. A case coordinator is responsible for problem identification, direct service, service coordination and discharge planning.

The CIA Specialist seeks to empower individuals, and help those individuals resolve problems or address issues which may be compromising their social functioning and/or impacting negatively on their quality of life. The scope of service is broad and might involve activities ranging from assistance in completing forms to intervening in circumstances where an individual's safety and well-being appear to be at risk.


Core Competencies

  • Experience in working with seniors and persons with a disability
  • Knowledge of municipal, provincial, and federal legislation pertinent to issues affecting seniors and persons with a disability
  • A demonstrated capacity to utilize sound judgment and maintain composure in difficult and crisis situations
  • Strong public speaking and presentation skills
  • Basic computer literacy skills
  • A valid driver's license and use of a personal vehicle

Job Duties

  • Receive community-based and self-referrals to the program
  • Complete a formal intake on all eligible referrals
  • Assess client needs and develop an appropriate plan for intervention/assistance based on apparent need and/or client goals
  • Work collaboratively with other service providers where appropriate to facilitate the achievement of key objectives within the healthcare system (e.g. providing advocacy and/or case management services where needed to facilitate an individual's transition back into the community from a long-term care home)
  • Provide education to clients, families, and community members regarding available community supports
  • Maintain client files in accordance with agency/program expectations
  • Contribute to community awareness regarding important issues relevant to the program (e.g. elder abuse)
  • Participate in job-related committee meetings such as Behavioural Supports Ontario (BSO), Sarnia-Lambton Elder Abuse Awareness Network (SLEAAN)
  • Perform other related duties as required

Requirements

  • Professional designation, such as Social Worker (preferred), Registered Practical Nurse, or Social Service Worker, or specialization may also be obtained through an acceptable combination of education, training, and/or experience
  • Completion of a satisfactory Criminal Reference Check and Vulnerable Sector Screen
  • Excellent oral and written communication skills and the ability to communicate effectively with individuals at all levels of the organization
  • Experience with internal databases required. Experience with AlayaCare is preferred

Working Conditions

  • Variable client housing conditions, which can include hoarding, strong odour situations, etc.
  • Travel may be required
  • Occasional overtime may be required
  • Sitting for extended periods of time
  • Manual dexterity required to use desktop computer and peripherals
